Block 1.
Enter the COMSEC Account No and name submitting the order.
Case 1.
Entrer le numéro et le nom du compte COMSEC à l’origine de la commande.
Block 2.
Enter the Transaction ID for the order. It includes the COMSEC Account No
of the MSK User Representative that is submitting the order, the current date in
YYYYMMDD format, and the one up sequence number for the current
calendar year.
Case 2.
Entrer l’ID de transaction de la commande, notamment le numéro de compte
COMSEC du représentant de l’utilisateur MSK qui présente la demande,
la date au format AAAAMMJJ et un numéro séquentiel correspondant à l’année
civile en cours.

Deux points (:) séparent les numéros de compte COMSEC.
Block 10.
Enter the end user’s six digit COMSEC Account No and their Privilege Certificate
Manager’s (PCM’s) six digit COMSEC Account No if key purpose is operational.
Enter 845550 as the PCM COMSEC Account No if the key purpose is test. A
colon separates the COMSEC Account No’s(: ).
Block 11.
Enter any remarks for that line item number.
Block 12.
Enter the COMSEC Account No of the account that the key will be shipped to.

Entrer toute remarque se rapportant à cette ligne d’article.
Block 13.
The ordering COMSEC Account MSK User Representative must sign the form.
Case 12.
