EFFECTS OF LONG-TERM DIENOGEST TREATMENT FOR ENDOMETRIOSIS
ON BONE MINERAL DENSITY

------------------------------------Introduction: Dienogest is a treatment option for endometriosis. Although serum estradiol concentration during dienogest
treatment may maintain at the appropriate level to prevent hypo-estrogenic side effects such as bone loss, data
regarding changes in bone mineral density (BMD) during dienogest treatment are still scarce, and mostly short-term. The
objective of this study was to evaluate the effects of long-term dienogest treatment for endometriosis on BMD.
Patients and Methods: This study included sixty-five reproductive-aged women who underwent conservative surgery for
endometriosis and used postoperative oral dienogest (2 mg/day) for at least 12 months to prevent the recurrence. BMDs
were determined by dual-energy X-ray absorptiometry before and after dienogest treatment.
Results: The mean age at starting dienogest and duration of dienogest treatment was 31.1 ± 6.9 years and 16.4 ± 5.1
months, respectively. BMDs declined significantly after dienogest treatment at the lumbar spine (-2.0%) and femoral
neck (-2.2%), but not at the total hip (-0.4%) compared to the baseline. However, BMDs were still within the expected
range for age after treatment. No correlation was found between BMD changes and age at starting dienogest at all sites,
and in addition, BMD changes were also not correlated with the duration of dienogest treatment.
Conclusion: This study suggests that long-term treatment of dienogest could have an adverse effect on BMD in
reproductive-aged women. Further high-quality clinical trial is warranted to establish the effects of long-term dienogest
treatment on bone mass.
