Adenomyosis shares some physiopathological mechanisms with endometriosis and should be considered when endometriosis is diagnosed. The aim of the study is to describe the association between women with intestinal nodules and ovarian endometriomas (OMA) or adenomyosis. Patients and methods: An observational study was designed in a tertiary universitary hospital from November 2014 to November 2015, including patients with suspected deeply infiltrating endometriosis (DIE). A transvaginal ultrasound with bowel preparation (TVUS-BP) was performed to all patients in order to detect intestinal infiltration of DIE. The presence of OMA and adenomyosis (considered when >2 ultrasound criteria were observed) was compared in patients with and without intestinal nodules of DIE. Results: A total of 132 patients were included in the study, with a mean age of 36.6 ± 5.2 years. Up to 51% of patients with OMA showed intestinal nodules, especially when ovarian cysts are bilateral (19 patients out of 28; p=0.01), whereas 42% of women with bowel involvement did not present ovarian affection. Additionally, 62% of patients with adenomyosis presented intestinal nodules, whereas only 37% of patients with bowel infiltration did not have criteria of adenomyosis in the TVUS (p=0.005). Therefore, the Odds Ratio (OR) to present rectosigmoid nodules when bilateral OMAs or adenomyosis are diagnosed is 2.96 (1.11-7-87; p=0.03) and 2.64 (1.13-6.20; p=0.02) respectively. When adherences of posterior DIE to the serosa layer of the rectum were also considered, the OR to present intestinal nodules increases to 3.89 (1.27-11.95; p=0.02) for bilateral OMAs and up to 13.96 (3.85-50.66; p=0.01) for adenomyosis. Conclusion: When bilateral OMAs are diagnosed, the likelihood of intestinal involvement is 3-fold compared with patients without ovarian affection. Moreover, the OR to present bowel affection is 2.6 when ultrasound criteria of criteria adenomyosis exist.
