This may result in impaired response to progestin. The effect of the association of vaginal gestrinone, a 19 nor-testosterone derivative, with oral resveratrol and Pinus pinaster extract on pain scores was investigated in these patients. Patients and Methods: Fifty patients with deep endometriosis in whom previous treatment had failed were enrolled for this study. Sixteen patients also had uterine fibroids and 42 also had adenomyosis. Pain was assessed at baseline and monthly during treatment. Blood analysis was performed after two months of treatment. Vaginal ultrasonography was performed at baseline and after three months of treatment. All patients were treated with vaginal gestrinone (5mg/ml) in Pentravan® thrice weekly with daily oral 30-mg doses of resveratrol and 100 mg of Pinus pinaster. Results: Vaginal gestrinone in Pentravan®, together with oral resveratrol and Pinus pinaster extract, significantly decreased the intensity of pain from a mean score of 9 at baseline to 0.4 by the second month of treatment. After three months, all patients were pain-free and amenorrheic. Uterine volume decreased significantly in patients with fibroids from 206 cm3 to 138 cm3 (p=0.001) by the third month of treatment and from 146 cm3 to 115 cm3 (p=0.04) in patients with endometriosis/adenomyosis. Ovarian endometriotic cyst volume also decreased significantly from 36 cm3 to 18 cm3 (p=0.004). Blood levels of estradiol, total testosterone and SHBG decreased significantly after two months (p=0.01). There was also a significant decrease in triglycerides, HDL cholesterol and glucose. Conversely, total cholesterol levels and LDL were not significantly affected. Liver enzymes did not increase significantly. Conclusion: The association of gestrinone with resveratrol, a histone deacetylase inhibitor, and Pinus pinaster, a NFKappa.b blocker, effectively treated endometriosis-related pain in patients who had failed to respond to previous treatment with progestins. ------------------------------------Mots clefs : endometriosis-related pain; vaginal gestrinone; resveratrol; Pinus pinaster extract.
