Specimen Collection and Preparation
Serum, plasma (EDTA, citrated, heparinized) and cerebrospinal fluid (CSF) can
be used as sample materials. All serum/plasma samples and controls
provided with the kit should be prediluted 1:20 with wash buffer; e.g. 10 µL
sample is mixed with 190 µL wash buffer. CSF can be used undiluted. At the
end of the procedure the final dilution of the sample is 1:2000.

During transportation, patient samples may be stored at room temperature for
up to 24 hours. For short term storage the samples can be stored at 2-8 °C for
up to 5 days. For long term storage the samples should be dispensed and
stored frozen at < -18°C. Avoid repeated freeze-thaw cycles.
Warnings and Precautions
All reagents of this test kit are strictly intended for in vitro diagnostic use only.
This test should be carried out only by persons who are familiar in performing
in vitro diagnostic procedures. Please follow strictly the sequence of pipetting
steps provided in this protocol.
Sample materials of patients are always potentially infectious. Handling of
samples suspected to be infectious should be done in a laminar flow clean
bench.

Method and Test Principle

The Onconeural Antigens IgG assay is an immunoassay in a dot blot format for
the qualitative detection of human IgG autoantibodies against the neuronal
antigens HuD, Ri, Yo and Amphiphysin. Human recombinant proteins HuD, Ri,
Yo and Amphiphysin are immobilized on fixed locations of a nitrocellulose
membrane; in addition anti-human IgG antibodies are immobilized as
functional control.
The prediluted sample is pipetted to the antigen coated carrier membrane.
During the first incubation period the autoantibodies present in the patient’s
samples will bind according to their antigen specificity. In the following step
unbound serum components are washed away. Bound immune complexes
are labelled with alkaline phosphatase conjugated anti-human IgG during the

next incubation period. Surplus of conjugate is removed by washing the strips.
Finally, immunocomplexes are visualized by the addition of the colorless
enzym substrate BCIP which forms a blue precipitate line. Type of antibodies in
the sample is then identified by comparing its position with the reference
bands on the evaluation sheet.

Test Performance
Do not interchange components of different lots and assays.
Do not use kit components beyond their expiration dates.
All components should be prewarmed at room temperature (18 – 28 °C)
before use.
Dilute concentrates at least 30 minutes prior to use. Mix well, but prevent
foam formation.
To avoid carryover contamination, carefully aspirate fluids; take
particularly care on patient sample containing fluids!

Overlay the strips with 1,000 µL wash buffer and incubate for 5 minutes at
room temperature (18-28 °C) on a shaker. Do not aspirate; wash buffer
remains in the reservoirs !

Add 10 µL prediluted patient serum/plasma and prediluted controls (resp.
50 µL undiluted cerebrospinal fluid) and incubate for 90 minutes on a
shaker.

Aspirate and wash the strips with 1,000 µL wash buffer. Do it by carefully
mixing and aspiration.

Add 1,000 µL of wash buffer and shake for 5 minutes. Aspirate the wash
buffer. Repeat this step once.
7.
Pipet 1,000 µL Enzyme Conjugate solution and incubate for 60 minutes
on a shaker.
8.
Aspirate and wash the strips with 1,000 µL wash buffer. Do it by carefully
mixing and aspiration.
9.
Add 1,000 µL of wash buffer and shake for 5 minutes. Aspirate fluid.
Repeat this step once.

10. Pipet 1,000 µL of Substrate Solution and incubate 5-15 minutes on a
shaker. Control the color development after 5 minutes. The functional
control should be a clearly visible band on all strips. Especially the
positive control should show five clearly visible bands.
11. Aspirate Substrate Solution and wash twice with 1,000 µL of distilled
water.

12. Remove the strips from their reservoir using tweezers. Airdry on adsorbent
paper for about 30 minutes.

After complete drying, the strips will be placed according their functional
control on the evaluation sheet (see interpretation sample), with the numbers
on top and the ends of the strips fixed with adhesive film. The position of the
antigen bands may slightly vary from lot to lot, but they always lay within the
indicated marks of the evaluation sheet. The blue color of the antigen bands
will fade, if the dry strips are exposed to air and light for a longer time.

bands with those of the positive control supplied with the kit. For that, the
positive control serves as cut-off control.

Quality Control
The functional control on each strip (internal reaction control) has to display an
intensive color. The negative control must show only the functional control
band; in very rare cases very faint bands can be seen, which have no
importance.

The positive control must always show an intensive staining of the four
antigens (Yo, Ri, Hu-D and Amphiphysin) and the functional control; if not, the
test has to be repeated.

Probenentnahme und -vorbereitung
Als Probenmaterial können Serum, Plasma (EDTA, Citrat, Heparin) und Liquor
cerebrospinalis verwendet werden. Alle Serum-/Plasmaproben und die im Kit
befindlichen Kontrollen werden 1:20 mit Waschpuffer vorverdünnt; dazu wird
beispielsweise 10 µL Probe zu 190 µL Wasch-Puffer gegeben. Liquor kann
unverdünnt eingesetzt werden. Am Ende der Testdurchführung ist die
Endverdünnung der Probe 1:2000.

Für den Transport kann das Untersuchungsmaterial bis zu 24 Stunden bei
Raumtemperatur gelagert werden. Die Proben können gekühlt bei 2-8 °C bis
zu 5 Tage aufbewahrt werden. Ist eine längere Lagerung beabsichtigt, sollten
die Proben aliquotiert und bei < -18 °C tiefgefroren werden. Wiederholte
Einfrier- und Auftauzyklen sind zu vermeiden.
Hinweise und Vorsichtsmassnahmen
Alle Reagenzien dieser Testpackung dürfen ausschließlich zur in vitroDiagnostik verwendet werden. Die Anwendung sollte durch Personal erfolgen,
das speziell in Verfahren von in vitro-Diagnostika unterrichtet und ausgebildet
wurde. Die Einhaltung des vorgeschriebenen Protokolls zur Durchführung des
Tests ist unbedingt erforderlich.

Untersuchungsmaterial von Patienten ist stets als potentiell infektiös

Methodik und Testprinzip

Der Onconeural Antigens IgG Test ist ein Immunoassay im Dot-Blot-Verfahren
zur qualitativen Bestimmung von humanen IgG-Autoantikörpern gegen die
neuronalen Antigene HuD, Ri, Yo und Amphiphysin. Die rekombinanten
humanen Proteine HuD, Ri, Yo und Amphiphysin sind auf einer NitrozelluloseMembran an fixen Stellen aufgetragen; zusätzlich sind anti-human-IgGAntikörper als Funktionskontrolle aufgetragen.
Die vorverdünnte Patientenprobe wird zu der Antigen-beladenen
Trägermembran pipettiert. Während der ersten Inkubation (90 Minuten) binden
die in der Patientenprobe vorhandenen Autoantikörper entsprechend ihrer
Antigen-Spezifität.

Einzelne Komponenten verschiedener Chargen und Testbestecke dürfen

Kitbestandteile dürfen nach Ablauf des Haltbarkeitsdatums nicht benutzt

Alle Komponenten müssen vor Gebrauch auf Raumtemperatur (18-28 °C)
erwärmt werden.
Die Konzentrate müssen mindestens 30 Minuten vor Gebrauch verdünnt
werden. Gut mischen, Schaumbildung vermeiden.
Um Kreuzkontaminationen zu vermeiden Flüssigkeiten vorsichtig
absaugen; das gilt vor allem für die Patientenproben-haltigen
Flüssigkeiten!

Serumbestandteile weggewaschen. Während der nächsten Inkubation (60
Minuten) werden die gebundenen Immunkomplexe mit anti-human-IgG, das
mit alkalischer Phosphatase konjugiert ist, markiert. Überschüssiges Konjugat
wird durch Waschen der Streifen entfernt. Schließlich werden die
Immunkomplexe sichtbar gemacht, indem sich durch Zugabe von farblosem
Enzymsubstrat (BCIP) eine blaue Präzipitat-Linie bildet. Die Antikörper-Typen
der Probe werden durch den Vergleich der Positionen mit denen der
Referenzbanden auf der Auswerteschablone identifiziert.

Streifen mit 1000 µL Waschpuffer überschichten und 5 Minuten bei
Raumtemperatur (18-28 °C) auf einem Schüttler inkubieren. Nicht
Absaugen, Waschpuffer verbleibt in den Reservoirs!

10 µL vorverdünntes Serum/Plasma und vorverdünnte Kontrollen (bzw. 50
µL unverdünnten Liquor) zugeben und 90 Minuten auf einem Schüttler
inkubieren.
5.
Absaugen und die Streifen mit 1000 µL Waschpuffer waschen. Dabei
vorsichtig schwenken und dann wieder absaugen.
6.
1000 µL Waschpuffer hinzugeben, 5 Minuten schütteln. Waschpuffer
absaugen. Diesen Schritt einmal wiederholen.
7.
1000 µL Enzymkonjugat-Lösung zupipettieren und 60 Minuten auf einem
Schüttler inkubieren.
8.
Absaugen und die Streifen mit 1000 µL Waschpuffer waschen. Dabei
vorsichtig schwenken und dann wieder absaugen.
9.
1000 µL Waschpuffer hinzugeben, 5 Minuten schütteln. Waschpuffer
absaugen. Diesen Schritt einmal wiederholen.

10. 1000 µL Substrat-Lösung zupipettieren und 5 - 15 Minuten unter Schütteln
inkubieren. Nach 5 Minuten die Farbentwicklung kontrollieren. Die
Funktionskontrolle sollte auf allen Streifen als intensive Bande sichtbar
sein. Insbesondere soll Positivkontrolle fünf deutlich sichtbare Banden
aufweisen.

11. Substratlösung absaugen und Streifen zweimal mit 1000 µL dest. Wasser
waschen.
12. Streifen mit einer Pinzette der Wanne entnehmen und für ca. 30 Minuten
auf Filterpapier lufttrocknen.
13. Das Bandenmuster mit dem im Kit vorhandenen Auswertebogen
interpretieren.

Die komplett getrockneten Streifen werden anhand der Funktionskontrolle auf
dem Auswertebogen (siehe Auswertebeispiel) mit der Nummerierung nach
oben ausgerichtet und mit Tesafilm an ihren Enden befestigt. Die Position der
Antigene auf den Streifen kann chargenabhängig geringgradig variieren, sie
liegen aber stets in dem Bereich der auf dem Auswertebogen befindlichen
Markierungen. Die blaue Färbung der Antigen-Banden verblasst, wenn die
trockenen Streifen Luft und Licht für längere Zeit ausgesetzt sind.

Siero, plasma (EDTA, citrato, eparinato) e liquido cerebrospinale (CSF)
possono essere utilizzati come materiale campione. Tutti i campioni di
siero/plasma forniti col kit devono essere prediluiti 1: 20 con tampone
lavaggio; ad es. 10 µL di campione è diluito con 190 µL di tampone lavaggio. Il
liquido cerebrospinale (CSF) può essere utilizzato non diluito. Alla fine di
esecuzione del test la diluizione finale del campione è 1:2000.
Durante il trasporto i campioni possono essere conservati a temperatura
ambiente fino a 24 ore. I campioni refrigerati at 2-8 °C si conservano fino a 5
giorni. Per periodi più lunghi i campioni vanno aliquotati e congelati a < -18°C.
Evitare ripetuti congelamenti e scongelamenti del campione.
Avvertimenti e precauzioni
Tutti I reattivi di questo kit sono tassativamente da intendersi solo per uso
diagnostico in vitro. Il kit va utilizzato esclusivamente da personale addestrato
all’uso di metodologie diagnostiche in vitro. Preghiamo di seguire alla lettera lo
schema di pipettaggio indicato in questa procedura.
I campioni dei pazienti sono sempre da considerarsi come potenzialmente
infetti. I campioni di pazienti potenzialmente infetti vanno maneggiati in banchi
di lavoro con cappe a flusso laminare.

Metodo e principio del test

Il test per gli Onconeural Antigens IgG è un immunoassay in formato “dot blot”
per la determinazione qualitativa di autoanticorpi IgG umani anti antigeni
neuronali HuD, Ri, Yo e Amfifisina. Proteine umane ricombinanti di HuD, Ri, Yo
e Amfifisina sono fissate su posizioni prestabilite su una membrana di
nitrocellulosa; inoltre anticorpi antii-IgG umane sono fissati sulla membrana
come controllo funzionale.
Il campione prediluito è pipettato sulla membrana di supporto su cui sono
fissati gli antigeni. Nel corso della prima incubazione gli autoanticorpi presenti
nel campione del paziente si legano secondo la loro specificità agli
antigeni.Nel passaggio successivo i componenti del campione non legati sono
eliminati col lavaggio. Durante il periodo di incubazione seguente gli
immunocomplessi legati vengono marcati con anticorpi anti-IgG umane,

coniugati con fosfatasi alcalina. L’eccesso di coniugato è rimosso con il
lavaggio delle strisce. Infine aggiungendo una soluzione di substrato incolore
(BCIP), gli immunocomplessi vengono visualizzati, con la formazione di una
banda di precipitato blu. Confrontando con le bande di riferimento sul foglio di
valutazione gli autoanticorpi presenti nei campioni vengono identificati.

Non mescolare componenti di lotti e dosaggi differenti.
Non utilizzare i componenti del kit dopo la data di scadenza.
Tutti i componenti devono essere portati a temperatura ambiente (18 –
28°C) prima dell’ uso.
Diluire I concentrati almeno 30 minuti prima dell’ uso Mescolare bene,
evitando la formazione di schiuma.
Per evitare contaminazione da trascinamento aspirare i liquidi con cura,
particolarmente i liquidi contenenti il campione!

agitatore. Controllaree dopo circa 5 minuti lo sviluppo del colore. La linea
del controllo funzionale deve essere chiaramente visibile in tutte le strisce.
In particolare il controllo positivo deve presentare cinque bande ben
visibili.

distillata.

Dopo che le strisce sono completamente asciugate, le stesse vengono
posizionate sul foglio di valutazione a partire dalla posizione del controllo di
funzionamento (v. esempio di interpretazione) con i numeri in cima alla parte
terminale della striscia fissata con pellicola adesiva. La posizione delle bande
degli antigeni può variare leggermente da lotto a lotto, ma le stesse si trovano
sempre in corrispondenza della posizione indicata sul foglio di valutazione. La
colorazione blu delle bande tende a sbiadire se le strisce asciutte sono
esposte all’aria e alla luce per un periodo prolungato.

Il controllo di funzionalità su ciascuna striscia (controllo di reazione interno)
deve mostrare un colore intenso. Il controllo negativo deve mostrare
solamente la banda di controllo interno; in casi molto rari possono essere
osservate bande molto deboli, senza nessun valore.
Il controllo positivo deve presentare sempre una colorazione intensa per i 4
antigeni (Yo, Ri, Hu-D e Amfifisina); altrimenti il test deve essere ripetuto.

Le sérum, le plasma (sous EDTA, citrate, héparine) et le liquide cérébrospinal
(CSF) peuvent être employés pour ce test. Tous les échantillons de sérum et
de plasma doivent être prédilués au 1:20 avec la solution de lavage; ex: 10 µL
d’échantillon mélangé à 190 µL de solution de lavage. Le CSF peut être utilisé
sans dilution. À la fin de la réalisation du test la dilution finale de l´échantillon
est 1:2000.
Pendant le transport, les échantillons peuvent être stockés à température
ambiante pendant 24 heures. Pour le stockage à court terme (5 jours
maximum), les échantillons peuvent être conservés à 2-8°C. Pour une
conservation à plus long terme, aliquotez les échantillons et congelez les à < 18°C. Evitez des cycles de congélation / décongélation répétitifs.
Avertissement et précautions d’emploi

Tous les réactifs de ce kit sont strictement réservés à un usage de diagnostic
in vitro. Le personnel doit être formé et exercer les bonnes pratiques de
laboratoire nécessaires au diagnostic in vitro (EN 375). Respecter
rigoureusement l'ordre des étapes de pipetage du protocole.
Les réactifs du kit doivent être considérés comme potentiellement infectieux.
Les échantillons issus de groupes de patients à risque doivent être identifiés et
manipulés sous hotte à flux laminaire.

Méthode et Principe du test

Le test Onconeural Antigens IgG est un immuno-test dot blot pour la détection
qualitative des auto-anticorps humains IgG anti antigènes neuronaux, HuD, Ri,
Yo et Amphiphysine. Des protéines humaines recombinantes HuD, Ri, Yo et
Amphiphysine sont fixées sur une membrane de nitrocellulose. Des anticorps
anti IgG humaines sont également fixés sur la membrane et servent de
contrôle interne.
L'échantillon prédilué est déposé à la pipette sur la membrane porteuse des
antigènes. Pendant la première période d'incubation (90 minutes) les autoanticorps présents dans l’échantillon du patient se fixent à leur antigène
spécifique. Le lavage permet d’éliminer les éléments non liés. Un conjugué

anti-IgG humaines couplé à de la phosphatase alcaline est ajouté et se fixe sur
les immunocomplexes liés à la membrane. L'excédent de conjugué est éliminé
par le lavage. En phase finale, les immunocomplexes sont visualisés par
l'addition d’un substrat incolore (BCIP) qui forme un précipité bleu en leur
présence. Les types d'anticorps présents dans l'échantillon sont alors identifiés
en comparant leur position aux bandes de référence sur la fiche d'évaluation.

Tous les composants doivent être à température ambiante (18 – 28 °C)

Diluer les concentrés moins de 30 minutes avant usage. Bien mélanger,
mais éviter la formation de mousse.

Ajouter 10 µL d’échantillon prédilué (sérum/plasma) ainsi que les
contrôles positif et négatif prédilués (remarque: ou 50 µL de liquide
cérébrospinal non dilué) et incuber 90 minutes sous agitation.

Aspirer le liquide et laver les bandelettes avec 1000 µL de solution de
lavage. Mélanger et aspirer soigneusement.
6.
Ajouter 1000 µL de solution de lavage et agiter 5 minutes. Aspirer la
solution de lavage. Répéter l’opération de lavage encore une fois.
7.
Ajouter 1000 µL de conjugué et incuber 60 minutes sous agitation.
8.
Aspirer le liquide et laver les bandelettes avec 1000 µL de solution de
lavage. Mélanger et aspirer soigneusement.

10. Ajouter 1000 µL de substrat et incuber 5-15 minutes sous agitation.
Vérifier le développement de la couleur après 5 minutes. Le contrôle
interne doit être clairement visible sur chaque bandelette. La bandelette
contrôle positif doit présenter 5 lignes colorées nettement visibles.
11. Aspirer le substrat et laver deux fois avec 1000 µL d’eau distillée.

12. Retirer les bandelettes du portoir en utilisant des pincettes. Laisser sécher
à l’air sur du papier absorbant pendant 30 minutes.

Après séchage complet, placer les bandelettes sur une feuille, numérotation
vers le haut, en alignant le contrôle interne. Fixer les extrémités des
bandelettes avec du ruban adhésif (voir l'exemple d'interprétation). La position
des bandes d'antigène peut légèrement varier d’un lot à l’autre, mais elles sont
toujours comprises entre les marques indiquées par la fiche d'évaluation. La
couleur bleue des bandes d'antigènes peut pâlir si les bandelettes sont
exposées trop longtemps à l'air et à la lumière.

intensités de couleur des bandes à celles du contrôle positif fourni avec le kit.
Pour cela, le contrôle positif sert de contrôle cut-off.

Contrôle Qualité
Le contrôle interne de chaque bande doit être de couleur intense. Le contrôle
négatif doit seulement présenter une coloration au niveau du contrôle interne;
dans de très rares cas des lignes d’intensité très faible peuvent être visibles,
sans aucune incidence sur le résultat.

Le contrôle positif doit toujours présenter des colorations intenses pour les 4
antigènes (Yo, Ri, Hu-D et Amphiphysine) et pour le contrôle interne, sinon le
test ne peut être interprété.
