FAST-TRACK COLECTOMY: OUTCOMES AFTER
IMPLEMENTATION OF A CLINICAL PATHWAY

Background and objective Following the publication of the ERAS
(Enhanced Recovery After Surgery) protocol by Kehlet’s group and a
preliminary test in our unit, we set up a clinical pathway in colon
surgery in order to assess clinical practices. The clinical pathway
was managed by a multidisciplinary team within the framework of
the V2 HAS accreditation procedure for health care organizations. A
French version of the ERAS protocol, adapted to the local context,
was drawn up. The entire care team was provided with information
and training relative to the protocol and to perimedullar analgesia
use. Our objective was to determine whether outcomes would be as
good as those we were obtaining without the use of the clinical
pathway (postoperative hospital stay for colon surgery reduced
from 12 days with ‘standard care’ to about 6.4 days). We wanted to
make sure that the clinical pathway would definitely reduce length
of hospital stay without increasing mortality or morbidity rates.
Programme From January 1, 2007, to December 31, 2008, we treated
147 patients with colorectal disease. Patients with colic cancer or
complicated diverticulosis scheduled for elective surgery (n¼62)
were enrolled in the clinical pathway. The ERAS protocol was
adapted to the entire population. In particular, we provided
comprehensive oral and written patient information, avoided
mechanical preparation of the bowel, offered perimedullar analgesia
whenever possible, and encouraged early postoperative feeding.
Results There were 28 women and 34 men (mean age, 67.3 years
(range 45e88)). Mean ASA score (American Society of Anesthesiologists) was 2. Mean Bone Mass Index was 27.5. Colectomy was
performed for cancer (n¼33) or diverticulosis (n¼20). Suspect
polyps were resected in 10 patients. Epidural analgesia (48 h) was
used in 33 patients. There were 35 left colectomies and 17 right
colectomies. Of the 11 laparotomies, four were conversions from a
laparoscopic procedure. The first passage of flatus occurred on
average after 1.8 days, the first stools on average after 2.35 days. One
patient died (1.6%). Morbidity was 20% (one anastomotic fistula
and one deep-seated abcess). Mean postoperative stay was 5.3 days
(median 4.0 days, range 2e26).
Discussion The introduction of a clinical pathway for fast-track
colectomy markedly reduced the length of postoperative stay
without increasing mortality and morbidity. Implementation of the

protocol enabled the application of the French Society of Digestive
Surgery’s (SFCD) 2005 good practice guidelines to the perioperative
care setting. Postoperative stay was reduced by 1.1 days.
This study helped the care team understand the principles and
advantages of practice evaluation. At the close of the study, the care
team drew up a specific written protocol for each disease treated in
the department, which gives a formal updated list of all technical
procedures. The document was amended and validated by the
medical team. It will hopefully be adopted by all the surgery
departments of our hospital.
Introduction Suite aux travaux de Kehlet et son protocole ERAS
(Enhanced recovery after surgery), ainsi qu’une expérience préliminaire informelle en chirurgie colique, nous avons mis en place un
chemin clinique formalisé servant de base à une EPP. Celle-ci
s’inscrivait dans le cadre de certification V2 et a réuni un groupe
multidisciplinaire et multiprofessionnel. Ce dernier a réécrit le
protocole ERAS en français et fixé les règles de fonctionnement. En
particulier une information/formation sur la méthode et le fonctionnement de l’ analgésie périmedullaire a été prodigué à l’ensemble
de l’équipe soignante. Nous voulions conforter notre expérience
initiale (séjour postopératoire en moyenne de 6,4 jours au lieu de
plus de 12 jours en moyenne avec une prise charge standard)) et
nous assurer que ce chemin clinique permettait de réduire de
manière certaine la durée de séjour postopératoire sans augmenter la
morbi-mortalité après colectomie en hôpital général.
Programme Du 01/01/200707 au 31/12/2008 nous avions pris en
charge 147 patients atteints de pathologie colorectale chirurgicale.
Pour des raisons d’homogénéité, seuls les patients vus de manière
élective et atteints de diverticulose compliquée ou de cancer du côlon
ont été sélectionnés. Soixante deux patients servent de base à ce
travail. Nous avons adapté pour tous les points, le protocole ERAS.
En particulier une information orale et écrite exhaustive a été
fournie. Il n’y a pas eu de préparation colique. L’analgésie péridurale
était proposée et réalisée si possible. Le jeûne était limité.
Resultats Il y avait 28 femmes et 34 hommes. L’âge moyen était de
67,3 ans (extrême 45-88). Le score ASA moyen était de 2. L’IMC
moyen était de 27,5; La colectomie a été faite 33 fois pour un cancer,
10 fois pour un ou plusieurs polypes suspects (exérèse carcinologique) et 20 fois pour une diverticulose. L’analgésie péridurale a été
faite 33 fois pour une durée de 48 heures. Il y a eu 35 colectomies
gauches et 17 colectomies droites. Il y a eu 11 laparotomies dont les
4 sont des conversions. La reprise du transit des gaz s’était fait en
moyenne en 1,8 jours. Pour les selles, le délai était de 2,35 jours en
moyenne. Il y a eu un décès (1,6%). La morbidité était de 20% (dont
une fistule anastomotique et un abcès profond). La durée moyenne
de séjour postopératoire a été de 5,3 jours (extrêmes 2-26, médiane 4
jours).
Discussion La mise en place d’un chemin clinique de réhabilitation
rapide pour les colectomies permet la réduction significative de la
durée de séjour sans augmentation de la morbi-mortalité. La
formalisation de cette démarche permet l’application des recommandations de bonne pratique dans les soins péri opératoires
(recommandations de la SFCD). Cela permet de voir la durée de
séjour postopératoire encore diminuée par rapport à l’expérience
initiale non formalisée (1,1 jours).
Ce travail par l’implication de l’équipe a permis de faire
comprendre les principes de l’évaluation et les bienfaits qu’elle
apporte. Ainsi à la suite de ce travail, les soignants se sont concertés
pour réaliser un document définissant pour chaque pathologie
traitée dans le service un protocole de prise en charge unique avec un
répertoire des gestes techniques actualisé et formalisé. Ce document
a été amendé et validé par l’équipe médicale .Il sera ensuite étendu
aux autres services du pôle de chirurgie de notre hôpital.

related outcomes, baseline results were already good. Globally, a
significant decrease in readmission rate (6.7% vs 15.7%, p<0.001)
was observed.
Benchmarking was a mean to improve the internal quality of
clinical hip fracture management processes and some related
outcomes in the three centers. In Nantes, thanks to the motivation
of the team, visible improvements were observed and measured.
Nurses and practitioners have decided to pursue the work: a
workgroup was constituted to achieve a clinical pathway of hip
fracture management process and a geriatrician was recruited soon
to take care of these old patients.
InPEC(H) was a program financed by the French Ministry of Health
to test the benefits of benchmarking practices of hip fracture’s
management. The objective was to assess whether comparison of
quality of hip fracture care is associated with improvement in
process and outcomes.
Three hospitals volunteered to participate in the project: two
teaching hospitals (Nantes and Rouen) and a big public one (Le
Mans). A multidisciplinary team modelled the different steps of the
hip fracture management process in each hospital. The three teams
agreed on a list of about 20 quality indicators including vital and
functional status 90 days after discharge. Baseline data were
collected on 150 consecutive patients per hospital, aged 65 years and
over, who received surgical treatment, between April and December
2003. The teams met to compare their results and initiate a
benchmarking process. They developed a specific improvement
program from January 2004. A new data collection of the same
indicators was achieved from May to December 2004 in the same
conditions in order to perform before-after comparisons. We report
the changes observed in Nantes teaching hospital.
Baseline data showed differing management patterns between
teams although patient age, autonomy and mobility score were
similar. Rate of surgery within 48 h of admission was 76% in Nantes
hospital (87% and 32% in the two other hospitals). The median
delay of the first postoperative stand-up was 4 days (2 and 4 days),
and 4 days for social worker intervention (3 and 7 days); the median
length of hospital stay in the orthopaedic unit was 10 days (9 and
14 days). The mean delay to send a discharge letter was 14.7 days
(SD:13.1) (2.1 and 1.2 days). 21.8% of patients were not satisfied
with information delivered about discharge (24.6% et 22.2%-NS).
During the 3 months following admission, 12% of the patients died
(12% and 17%dNS) and 5.5% were readmitted (18% and 14.5%d
NS). After comparison, a significant improvement was observed in
10 areas, seven of which corresponded to quality improvement areas
chosen for improvement action by professionals. At Nantes,
benchmarking allowed orthopaedic team to pick up a teaching
program for nurses being able to initiate an earlier postoperative
stand up. This was the first objective of an improvement program
aiming also to reduce the delay of social worker intervention and of
sending the discharge letter, and to improve patient satisfaction
about discharge information. The before-after comparison showed a
significant improvement for the 4 indicators: the first postoperative
stand up delay became 3 days (p<0.01), the social worker intervention delay became 2 days (p<0.001) and the mean discharge
letter delay became 12.2 days (SD:20.4dp<0.05). Patient dissatisfaction related to discharge information was reduced to 8.9%
(p<0.01). There was no significantly change in readmission rate,
mortality, functional outcome or length of stay, but for all these

tester une démarche de benchmarking pour améliorer la qualité de la
prise en charge de la fracture de l’extrémité supérieure du fémur
(FESF).
Trois hôpitaux étaient volontaires pour participer au projet: 2
hôpitaux universitaires (Nantes et Rouen) et un hôpital public (Le
Mans). Un groupe de travail pluriprofessionnel a décrit le processus
et les étapes importantes de prise en charge des FESF. Un consensus
sur une liste commune d’environ 20 indicateurs incluant le statut
fonctionnel et la mortalité à 3 mois après la sortie a été obtenu. La
phase de recueil des données s’est déroulée entre avril et décembre
2003: 150 patients par centre, âgés de 65 ans et plus, opérés d’une
FESF ont été inclus. Une réunion de confrontation des équipes a
permis d’analyser les écarts et d’initier une démarche de benchmarking. Chaque équipe a ensuite mis en œuvre un programme
d’actions d’amélioration spécifique à partir de janvier 2004. Afin de
mesurer l’évolution, une nouvelle phase de recueil des indicateurs
s’est déroulée dans chaque centre entre mai et décembre 2004 dans
les mêmes conditions. Les améliorations observées au CHU de
Nantes sont présentées dans les résultats.
L’évaluation initiale a montré des différences significatives de
pratiques entre les équipes alors même que l’âge moyen, l’autonomie
et la mobilité des patients ne diffèraient pas entre les centres. Le délai
opératoire <48H était de 76% à Nantes (87% et 32% dans les 2
autres centres). La date médiane du premier lever était de 4 jours (2
et 4 jours) et le délai médian de première demande sociale de 4 jours
(3 et 7 jours); la durée médiane d’hospitalisation était de 10 jours (9
et 14 jours). Le délai moyen d’envoi du courrier au médecin traitant
était de 14.7 jours (ET:13.1) (2.1 et 1.2 jours). 21.8% des patients
n’étaient pas satisfaits de l’information délivrée sur leur sortie
(24.6% et 22.2%-NS). A 3 mois, le taux de décès était de 12% (12% et
17% - NS) et 5.5% étaient réadmis (18.0% et 14.5%-NS). La
comparaison avant-après a montré une évolution significative dans
10 domaines, et pour 7 d’entre eux, les professionnels ont mis en
place des actions d’amélioration. A Nantes, un programme d’actions
a été mis en place: réduction du délai de premier lever et du délai de
prise en charge sociale, réduction du délai d’envoi du courrier au
médecin traitant et amélioration de l’information des patients
concernant leur sortie. La comparaison avant-après a permis de
montrer une amélioration significative dans les 4 domaines: délai
médian de premier lever réduit à 3 jours (p<0;01), délai médian de
prise en charge sociale à 2 jours (p<0.001) et délai moyen d’envoi du
courrier moyen à 12.2 jours (ET:20.4-p<0.05). L’insatisfaction des
patients vis-à-vis de l’information délivrée à la sortie a été réduite à
8.9% (p<0.01). Le taux de réadmission, de taux de mortalité et l’état
fonctionnel à 3 mois ainsi que la durée de séjour n’ont pas significativement évolué, mais les résultats initiaux étaient satisfaisants.
Globalement, il existait une diminution significative du taux de
réadmission à 3 mois (15.7% vs 6.7%-p<0.001)
Le benchmarking entre équipes a permis d’améliorer de façon
significative le processus de prise en charge de la FESF. A Nantes, des
améliorations visibles ont été obtenues grâce à la motivation de

Background and objectives The Centre Hospitalier Chrétien (CHC)
Liège (Belgium) consists of six clinics (1051 acute care beds, 30
psychiatric beds, 564 beds in the network for elderly patients).
These clinics implement Clinical Pathways (CP) using a methodology for 30 activities adapted from the Deming Wheel. The
methodology is taught in the CP network of which CHC is part
since the creation of the network in December 2003. The CP for
Total Knee Arthroplasty (TKA) on the St-Joseph site was the first CP
to be implemented by CHC (since December 4, 2004). In 5 years,
448 patients have followed this CP.
Pain is a key clinical indicator in all types of surgery. However,
before the introduction of CPs, pain assessment was not performed
in the surgical unit performing TKAs. An objective of our CP was to
achieve a pain score < or¼3 on a numerical scale ranging from 1 to
10 on transition to oral analgesia (ie, on the 3rd postoperative day).
Programme description, implementation, and follow-up The following
steps were implemented with regard to pain management: (i)
Information and training of the multidisciplinary team in pain
scoring (principles, myths, value, etc), (ii) Introduction of pain
assessment at rest and during mobilisation throughout the patient’s
hospital stay (minimum 3 times/24 h, (iii) Development of an
evidence-based protocol for pre-, peri- and postoperative medical
care (15% patient controlled analgesia (PCA), 85% patientcontrolled peripheral nerve block (PCPNA) during postoperative
care), (iv) Implementation of the CP, (v) Inclusion of information on
pain management in the patient CP brochure.
Results At 1 year, use of pain scoring was optimal in our unit and
the objective seemed to have been reached when considering average
pain scores. The average score out of 10 for PCA was 2.33 [3.5]* on
D3 during rest and 1.67 (2.5) on D3 during mobilisation (*(P75)).
The average score for PCPNA was 1.97 (2.75) on D3 during rest and
2.74 (4) during mobilisation. However, the pain score remained > 3
in 25% of the patients (P75). A multidisciplinary discussion of the
findings thus took place. The care protocol was reviewed and
improved. Patient information was also improved by producing a
brochure on management of acute pain.
In terms of clinical impact, we then observed:
< More active patient participation in pain assessment and
management
< Objective achieved at 4 years and maintained ever since. The
mean pain scores (out of 10) at 5 years were:
– PCA: D3 at rest¼0.11 (0) D3 during mobilisation¼0.46 (0)
– PCPNA: D3 at rest¼0.37 (0) D3 during mobilisation¼1.05 (2)
< Increased patient and team satisfaction according to satisfaction surveys.
Discussion and conclusion Pain management requires that all health
care professionals increase their awareness of pain, change their

attitude, and invest themselves at all times. The use of pain scores is
essential in order to encourage teams to move forward and in order
to provide visibility to the rest of the institution. Our CP procedure
was rapidly adopted for all patients regardless of the indication for
surgery. The decision to score pain in order to improve pain
management has convinced others, and its use is spreading
throughout the institution. From December 2009, the booklet on
the management of acute pain will be given and explained to all
patients before elective surgery on all CHC sites.
Contexte, objectifs Le Centre Hospitalier Chrétien (CHC) regroupe 6
cliniques, soit 1051 lits aigüs, 30 lits de soins psychiatriques et 564
lits de réseau sénior, dans la région liégeoise. Les itinéraires cliniques
(IC) y sont implémentés selon la méthodologie* enseignée au Réseau
des Itinéraires Cliniques (RIC) dont le CHC fait partie depuis sa
création en décembre 2003.
*méthodologie en 30 activités adaptée de la roue de Deming
L’IC prothèse totale de genou (PTG) sur le site St Joseph est le 1er
IC du CHC. Il est effectif depuis le 04/12/04 (en 5 ans ¼ 448
patients pris en charge selon l’IC).
Pour toute pathologie chirurgicale, la douleur (D+) représente un
indicateur clinique de choix.
Dans notre IC, un des objectifs a été d’obtenir un score de douleur
< ou ¼ 3 sur une échelle numérique de 1 à 10, lors du passage à
l’analgésie per os (soit au 3ième jour post-opératoire).
Description, mise en œuvre, éléments de suivi Avant l’IC, l’évaluation
chiffrée de la douleur était inexistante dans le service de chirurgie
concerné.
Les principales étapes réalisées autour de la prise en charge de la
douleur ont été:

Discussion, conclusion La prise en charge et la gestion de la douleur
nécessitent une prise de conscience, un changement de mentalités de
la part de tous les intervenants et un investissement de tous les
instants. Disposer de données chiffrées s’avère indispensable pour
stimuler l’équipe à progresser et permet également une visibilité
pour le reste de l’institution.
La démarche instaurée dans le cadre de l’IC PTG s’est rapidement
étendue à tout patient du service de chirurgie quelle que soit la
pathologie rencontrée. Elle fait également « tâche d’huile » à tous les
niveaux de l’institution. En effet, les preuves « chiffrées » de l’amélioration de la prise en charge de la douleur en ont convaincu plus
d’un. Dès décembre ’09, la brochure concernant la prise en charge de la
douleur aigüe sera donnée et expliquée en pré op à tout patient devant
subir une intervention chirurgicale, quel que soit le site du CHC.

Introduction The aim of the study was to assess the impact of a
clinical pathway implemented in a post-anaesthesia care unit
(PACU) on postoperative outcomes.
Methods A patient clinical pathway which includes a nurse-driven
fast track program (FTP) for uncomplicated patients and minor
interventions and a medical-driven slow track program (STP) for
complicated patients were implemented over a period of 2 months.
A retrospective cohort study based on electronic patient records was
performed to assess the effectiveness of this program on PACU and
hospital length of stay (LOS) and in-hospital mortality and
unplanned admission to the intensive care unit (ICU). We compared
the period before and after implementation of the program. Statistical analysis was performed using the Student t test, c2,
Wilcoxon-rank test. Multivariate analysis using linear and logistic
regression was performed to adjust for differences in patient
demographic characteristics, co-morbidities, type of surgery and
anaesthesia and emergency status between the two periods.
A p#0.05 was considered significant.
Results Patients characteristics, ASA scores, type of anaesthesia and
surgery were similar before and after implementation of the clinical
pathways. We observed a decrease in PACU median length of stay
for all patients from 163 min (IQR, 103e291) to 148 min (IQR,
96e270 min) (p<0.001). In ASA 1-2 patients median LOS in PACU
decreased from 152 min (IQR, 102e249 min) to 135 min (IQR
91e227 min) in ASA 1-2 patients (p<0.001). Hospital LOS however
remained unchanged. Overall in-hospital mortality after pathway
implementation decreased for all patients from 1.7 to 0.9% (adjusted
OR 0.36 (95%CI 0.22 to 0.59), p value<0.01). The number of
unplanned admissions to ICU decreased also from 113 (2.8%) to 91
(2.1%) (adjusted OR 0.73 (95%CI 0.53 to 0.99); p value 0.04).
Conclusion A significant reduction of LOS in PACU, in-hospital
mortality and unplanned admission to ICU was observed after
implementation of a clinical pathway in the PACU.

Introduction La phase postopératoire est une phase risquée pour le
patient en terme de complications. La mortalité postopératoire
actuelle, tous actes chirurgicaux confondus, se situe entre 1.2 et
2.6%, alors que la morbidité est supérieure à 11%.
Dans notre salle de surveillance post-interventionnel (SSPI), nous
avons un flux important de patients en bonne santé habituelle ayant
bénéficié d’un acte chirurgical mineur et des patients polymorbides
ayant bénéficié d’une chirurgie majeure. Nous avons mis en application un programme de prise en charge des patients permettant
de libérer rapidement ceux en bonne santé habituelle sur la base de
critères cliniques standardisés de manière à libérer les ressources
de soins nécessaires pour la prise en charge des patients à haut risque
de complications.
But de l’étude Le but de cette étude était d’évaluer les bénéfices d’un
programme de qualité incluant deux itinéraires cliniques distincts de
prise en charge en SSPI en terme de durée de séjours en SSPI, de
mortalité postopératoire et de complications postopératoires sévères
nécessitant une admission aux soins intensifs.
Description du programme et Méthode Le programme de prise en
charge des patients comprend deux itinéraires distincts. Le premier
itinéraire consiste en une évaluation et libération des patients en
bonne santé habituelle par le personnel infirmier de la SSPI sur la
base de critères cliniques stricts (FTP: fast-track pathway); le second
itinéraire, qui s’applique aux patients polymorbides ou ayant bénéficié d’une chirurgie majeure, consiste en une évaluation et prise en
charge médicale (STP: slow-track pathway).
Nous avons rétrospectivement inclus dans notre étude tous les
patients adultes ($18 ans) admis en SSPI ayant bénéficié d’un acte
chirurgical ou de médecine interventionnelle sous anesthésie. Les
informations principales ont été obtenues à partir des données
administratives contenues dans la base de données anesthésique.
Les périodes de l’étude étaient: du 01.01.2007 au 31.07.2007 (avant
l’implémentation du programme) et du 01.01.2008 au 31.07.2008
(après l’implémentation du programme). Afin de compléter la
collecte, les bases de données administratives de l’hôpital (DPI) ont
été utilisées, notamment pour l’obtention de données telles que la
mortalité ou les durées de séjour. Elles ont été jointes aux données
initiales. Toutes les données une fois collectées ont été anonymisées.
Résultats Les données démographiques, scores ASA, type de chirurgie ont été similaires dans les 2 périodes incluant environ 9’000
patients. La durée de séjour médiane en SSPI a diminué pour
l’ensemble des patients passant de 163 (IQR 107-291) en 2007 à 148
minutes en 2008 (IQR 96-270) après l’implémentation du
programme (p<0.001); cette diminution a été plus marquée pour les
patients ASA 1 et 2 passant de 152 (IQR 102-249) à 135 minutes
(IQR 91-227) (p<0.001). La mortalité après séjour en SSPI a diminué
de 68 (1.7%) à 39 (0.9%) et cette diminution s’est révélée significative même après correction pour l’âge, le type de chirurgie, le
type d’anesthésie et les interventions en urgence (OR ajusté 0.36;
IC95% 0.22 to 0.59; p<0.01). Pour les patients ASA 3-5, la baisse de
mortalité est de 57% après l’implémentation du programme.Le
programme a permis une réduction significative des complications
postopératoires sévères nécessitant des admissions imprévues aux
Soins Intensifs, faisant passer celui-ci de 113 (2.8%) à 91 (2.1%) (OR
ajusté; IC95% 0.53 to 0.99; p¼0.04).
Conclusion L’implémentation d’un programme de prise en charge des
patients incluant deux itinéraires distincts en phase postopératoire
immédiate a permis une diminution de la durée de séjour en SSPI,
une baisse considérable de la mortalité postopératoire ainsi qu’une
réduction du taux d’admissions imprévues aux Soins Intensifs après
un séjour en SSPI.

Background and objectives Antibiotic prophylaxis is a major tool for
the safe implantation of any medical device. The ‘Société Française
d’Anesthésie et Réanimation’ (SFAR) produced the first French
guidelines for antibiotic prophylaxis back in 1992 but, since then,
several practice surveys have shown that compliance is low. Our
objective was to develop recommendations for improving compliance.
Programme We reviewed 153 analytical questionnaires that had
been completed by all the orthopaedic surgeons who had reported a
risk event related to antibiotic prophylaxis during their 2008
‘accreditation programme’. Reporting was to Orthorisq, a certified
organisation. The questions explored four fields directly related to
the circumstances and professional conditions prompting the report:
(1) characteristics of the institution’s antibiotic prophylaxis
protocol, (2) circumstances leading up to the discovery of the event,
(3) search for human and organisational causes, and (4) recovery
actions, if carried out. The questionnaires reflected the experience of
orthopaedic surgeons, most of whom were discovering risk
management procedures for the first time.
Results An antibiotic prophylaxis protocol existed in nearly all
institutions, but was not available at the appropriate site in 15% of
cases. Most orthopaedic surgeons considered that the protocol was
suited to their practice, but over 10% did not know its exact
content. Most reports were the result of an immediate peri-operative dysfunctional event that the surgeon had discovered by
chance. Most risk events were discovered after skin incision.
A systematic prospective (use of checklist) or retrospective (audit)
analysis was very seldom performed. The causes of the event were
material (28%), human (92%) and/or organisational (50%). The
main cause of inappropriate antibiotic prophylaxis was ‘omission of
administration through negligence or inadvertence’ (56% of reports).
Overall, 65% of surgeons reported a low-grade recovery procedure,
but only 20% of them were able to reinitiate an appropriate antibiotic prophylaxis procedure.
Discussion and conclusion Our results have highlighted the risks
related to the cross-disciplinary and cross-professional (‘transversal’)
aspects of the prescription and administration of antibiotic
prophylaxis, thus raising the issue of the practitioner ’s responsibility in the case of a nosocomial infection. Because of the high rate
of omissions, 40% of surgeons spontaneously suggested routine use
of a checklist. The following improvement axes were proposed: (1)
updating protocols for antibiotic prophylaxis by indicating ‘who
does what?’; (2) routine checks of protocol knowledge by all new
arrivals (resident practitioners, temporary staff, and locums); (3)
involving patients in data collection for their medical records; (4)
encouraging use of a checklist in line with WHO and HAS recommendations. These conclusions should be taken into account when

SFAR next updates its guidelines on antibiotic prophylaxis. They
could also be used as an information source for medical professional
liability insurance purposes in order to define professional
malpractice profiles better.
Contexte, objectifs L’antibiothérapie prophylactique est un des
atouts majeurs à la pratique sécurisée de l’implantation de tout
dispositif médical implantable. La Société Française d’Anesthésie et
Réanimation (SFAR) a édicté en 1992 les premières recommandations en antibioprophylaxie, mais les différentes enquêtes de
pratique montrent une observance faible.
Programme Ce travail est basé sur l’étude de 153 grilles d’ « analyse
approfondie » ciblée adressées aux chirurgiens orthopédistes ayant
déclarés un événement porteur de risque (EPR) concernant l’antibioprophylaxie au cours de l’année 2008 dans le cadre de leur « bilan
d’accréditation » via l’organisme agréé Orthorisq. Les questions
proposées exploraient quatre domaines intéressant directement les
circonstances et l’environnement professionnel ayant conduit à la
déclaration du dysfonctionnement: (1) les caractéristiques du
protocole d’antibioprophylaxie de l’établissement, (2) les circonstances de découverte de l’événement, (3) la recherche de causes
humaines et organisationnelles et (4) les actions de récupération
menées quand elles existaient. Ces questionnaires représentent un
retour d’expérience des chirurgiens orthopédistes découvrant, dans
leur grande majorité, la démarche de gestion de risques.
Résultats Un protocole d’antibioprophylaxie existe dans quasiment
tous les établissements mais n’est pas disponible aux endroits
adéquats dans environ 15% des cas. La majorité des chirurgiens
orthopédistes considère qu’il est adapté à leurs pratiques mais plus
de 10% ne savent pas ce qu’il contient. La majorité des déclarations
provient de découvertes fortuites par le chirurgien d’un dysfonctionnement péri-opératoire immédiat. Une analyse systématique,
prospective (check list) ou rétrospective (audit), reste anecdotique.
Des causes humaines ont été retrouvées dans 92% des déclarations,
des causes matérielles dans 28% et des causes organisationnelles
générales dans 50% des déclarations. Quant aux actions de récupération, si 65% des déclarants disent avoir mis en place une
procédure dégradée, seuls 20% ont pu réinstaurer une procédure
d’antibioprophylaxie conforme.
Discussion Les résultats de cette enquête mettent clairement en
évidence les risques liés au caractère transversal de la prescription et
de l’administration de l’antibioprophylaxie, soulevant le problème de
la responsabilité des praticiens en cas d’infection nosocomiale. La
majorité des EPR ont été découverts une fois l’incision déjà réalisée.
La principale cause d’antibioprophylaxie mal ou non réalisée reste
« l’oubli de l’administration par négligence ou inadvertance » (56%
des déclarations): dans cette optique, l’institution d’une « check list
» était spontanément proposée par 40% des déclarants.
Conclusions Les axes d’amélioration proposés sont: (1) la rédaction de
protocoles d’ATBP précisant « qui fait quoi? »; (2) la vérification de la
bonne connaissance de ces protocoles par les nouveaux praticiens,
vacataires ou remplaçants; (3) l’implication du patient dans la
collection des informations médicales le concernant; (4) l’institution
de la pratique d’une « check list » conformément aux recommandations de l’OMS et de la HAS. Ces conclusions mériteraient d’être
prises en compte lors de l’actualisation des recommandations sur
l’antibioprophylaxie prévue par la SFAR. Enfin, elles pourraient
participer à l’établissement de recommandations susceptibles
d’intervenir dans l’évaluation des profils de sinistralité des praticiens.

Background and objectives Since 2003 we have been prescribing
fondaparinux sodium (SF) for the prevention of postoperative
thromboembolism. It is the most effective drug for femoral neck
fracture (FNF) but carries a high risk of bleeding.1 Many orthopaedists have discontinued its use because of severe postoperative
wound haematomas. In 2007, we performed a targeted clinical audit
(‘SF and FNF’) of 65 files2 which revealed that: (i) creatinine clearance (CC) was calculated (Cockcroft and Gault formula) in 7.7% of
cases only, (ii) 8.5% of SF prescriptions were not adapted
(CC#29 ml/min), (iii) 7.9% of first injections were administered
before the 6th postoperative hour, (iv) 55.6% of patients with
moderate chronic renal failure received SF before the 8th postoperative hour, (v) 5/65 patients experienced severe bleeding.2 3 In
2008, the medical team was informed of these findings and the
protocol was modified. SF was to be prescribed to patients with
>60 ml/min CC and the first injection had to be administered the
day following the operation at 6:00.4 This gave the objectives of the
second audit.
Programme The audit concerned patients who underwent lower
limb surgery and received SF between September 2008 and January
2009). The data analysed were: gender, age, appropriateness of the
prescription, time of first injection, and severe bleeding complications.

CC et la première injection devait être réalisée le lendemain à 6h.4
Donc les objectifs et le but du deuxième audit étaient définis.
Programme Etaient inclus tous les opérés du membre inférieur sous
FS de septembre 2008 à janvier 2009. Les données analysées étaient
le sexe ratio, l’âge moyen, l’adaptation des prescriptions, l’heure de la
première injection et les complications hémorragiques graves.

Discussion and conclusion Points of note are the small number of
patients in the second audit (three patients were operated twice),
their lower mean age, the inverse sex ratio, and the variety of types
of lower limb surgery. After the first audit, the prescription rate
decreased but two out of four anaesthesiologists continued to
prescribe FS. There was a marked improvement in the CC calculation rate. Overall, 39% of patients received the first injection at
6:00. and no haematomas occurred. In 2007, the French Health
Products Safety Agency (AFSSAPS) recommended FS injection 8 h
after surgery which means that 10% of patients in fact received FS
too early.
In conclusion, the results of the first audit prompted an
improvement in practices. CC calculation enabled adjustment of the
FS prescription. There have been no complications.

Contexte et objectifs Depuis 2003 nous prescrivons du FS en
prévention de la maladie thromboembolique post opératoire. Dans la
fracture du col fémoral (FCF) il est le plus efficace mais avec un
risque hémorragique plus important.1 Devant d’importants hématomes des plaies opératoires, les orthopédistes ne souhaitaient plus
qu’il soit utilisé.
En 2007 l’audit clinique ciblé « FS et FCF »2 sur 65 dossiers, a
montré que:
Discussion Petit collectif de 25 patients pour 28 dossiers (3 patients
réopérés), d’âge moyen plus bas avec un sexe ratio inversé, des
pathologies différentes mais toujours sur le membre inférieur. Après
le premier audit, la prescription a diminué, 2 anesthésistes sur 4 ont
continué à prescrire. Nette amélioration du calcul de la CC 52%
contre 7,7%. 39% des patients ont reçu la première injection à 6
heures le lendemain, pas d’hemorragie. L’Afssaps (2007) conseillait
d’injecter le FS 8 heures après la chirurgie, donc 10% des patients
l’ont reçu trop tôt. Suite au premier audit les soignants avaient pris
conscience des écarts de pratique, et ils ont modifié leurs pratiques.
Conclusion Le calcul de la CC par les MAR a permis, notamment, la
prescription adaptée du FS. Les pratiques ont évolué et les complications ont disparu.

par une anémie deux jours consécutifs.
Les saignements chirurgicaux avant (P1) et après mesures
correctrices (P2) ont également été mesurés.
Résultats Les taux de retard transfusionnel après chirurgie orthopédique chez le vieillard pour l’année précédant les mesures correctrices
et les trois suivantes étaient respectivement de: 22, 16, 9 et 15%
(p¼0,048) pour un seuil défini à 9 g/dl d’Hb et 3,2, 4,4, 1,4, 1,6% (ns)
pour un seuil défini à 8 g/dl d’Hb. Il n’existait pas de variation
statistiquement significative du saignement chirurgical entre P1 et P2.
Discussion Le suivi biologique collectif du saignement postopératoire
en orthopédie peut être recueilli « à la sortie de l’automate »
et constituer un outil de mesure de l’efficacité de la maitrise du risque
d’anémie postopératoire.

Background Inadequate management of blood losses is one of the
leading cause of postoperative morbidity.1 The most commonly
involved surgery is orthopaedic surgery and the main issue seems to
be delayed transfusion.2 Having validated the relevance and feasibility of a querying method,3 we studied the variations of indicators
extracted from biological databases according to corrective measures
aiming to detect and correct postoperative anaemia.
Methods The corrective measures were: (1) The definition and the
communication of transfusion thresholds during preoperative evaluation (2) An early telephone warning from laboratory in case of
anaemia. The chosen indicator was the incidence of elderly patients
sustaining delayed transfusion defined by a haemoglobin (Hb) level
beyond transfusion threshold on two consecutive dosages two
distinct days.
Surgical blood losses before (P1) and after corrective measures (P2)
were also compared.
Results The incidences of delayed transfusion in the elderly after
orthopaedic surgery the year before and the 3 years following
corrective measures were respectively: 22, 16, 9 and 15% (p¼0.048)
for a transfusion threshold of 9 g/dl Hb and 3.2, 4.4, 1.4, 1.6% for a
transfusion threshold of 8 g/dl Hb (ns). There was no difference
between P1 and P2.
Discussion A population based follow-up of postoperative haemorrage can be drawn by monitoring electronic databases. This allows
an indicator of control of postoperative anaemia.

fréquemment concernée est l’orthopédie et le principal problème en
cause semble être le retard transfusionnel2 Après avoir validé la
pertinence et la faisabilité d’une méthode d’interrogation,3 nous
avons étudié les variations des indicateurs fournis par les bases de
données issues des laboratoires de biologie en fonction des mesures
destinées à détecter et traiter l’anémie postopératoire.
Matériel et méthodes Les mesures correctrices étaient: (1) la définition et la communication de seuils transfusionnels dés la consultation d’anesthésie (2) la communication téléphonique précoce des
anémies par le laboratoire. L’indicateur choisi était le taux de retards

Setting Surgical antibiotic prophylaxis is one of the critical tools to
prevent Surgical Site Infections (SSIs) and needs, for its optimisation, to be in accordance with specific guidelines. Antibiotic
consumption in the operating room reaches almost 30% of the total
hospital antibiotic expenses and thus is a major goal in the way to
improve the overall antibiotic use. The implementation of personalised surgical antibiotic prophylaxis kits (SAPKs) has been associated with an improvement of the compliance to the guidelines
(82% of consistency).1 Nevertheless, these kits are associated with a
additional work load impairing their use in daily practice. The aim
of the process presented here is to move from personalised to SAPKs
« by type of surgery » in the way to make the implementation of
SAPKs easier and sustainable without impairing its efficiency.
Program Kits are made at the Hospital Pharmacy (in accordance
with the French Society of Anesthesiology (SFAR) Guidelines. They
are individual small paper bags including the labelling of the kit and
the antibiotic (name and dose, how to use it). The allocation of the
all week antibiotics is renewed every Friday by the Hospital Pharmacy (full-empty replacement using a rolling closet located in the
recovery room). To assess the impact of this process we designed an
exposed-non exposed prospective study (Visceral and Orthopaedic
Surgeries) defining two groups: non exposed patients having a freely
prescribed surgical antibiotic prophylaxis (fpSAPKs) versus « by type
of surgery » (tsSAPKs).
Results 420 patients have been included. The two groups are
comparable regarding the type of surgery (Orthopaedic 33%,
Visceral 66%), the age, gender, ASA and Altemeier classes and
duration of surgery. A surgical antibiotic prophylaxis has been

prescribed in 88% of cases in the fpSAPKs versus 90% in tsSAPKs
(p¼ns). The assessment of the main points of antibiotic prophylaxis
use showed a significant improvement of the consistency of the
reason of the prescription (93% vs 86%, p¼0.01) and of the chosen
drug (88% vs 81%, p¼0.04). The only one criteria not improved was
the delay between the time of antibiotic infusion and the beginning
of surgery (organisational constraints). The routine use of antibiotic
kits allowed to remove any antibiotic storage in the operating room
and to improve the medical prescription’s follow up through a
computerised information system. With the kits, the number of
different antibiotics used in the operating room decreases from 28 to
8. The complete accordance to the guidelines increases from 75% to
87% in the tsSAPKs group (p<0.01).
Discussion The implementation of SAPKs « by type of surgery »
allowed to improve the level of conformity to the guidelines. This
sustainable process (since 2005) ensures the mandatory (legal issue)
follow up of the prescriptions of antibiotics.
Contexte L’antibioprophylaxie (ABP) est un des moyens essentiels
de prévention des infections du site opératoire. La consommation
d’antibiotique au bloc opératoire représente près de 30% de la
consommation hospitalière totale et de ce fait représente un enjeu
important du bon usage des antibiotiques. La mise en place de kits
d’ABP individualisés par patients a montré son efficacité pour
l’application des référentiels (82% de conformité).1 Mais ces kits
sont une charge de travail importante, empêchant leur utilisation
en routine. Le but de cette étude est de proposer une évolution
vers des kits par indication chirurgicale, permettant une implantation et une perennité de la démarche sans en perdre le bénéfice
initial.
Programme Les kits ont été réalisés par la pharmacie hospitalière
(référentiel SFAR), en sachets comportant l’intitulé du kit et de
l’antibiotique (nom et dose, modalités). La dotation des kits est
renouvelée par la Pharmacie Hospitalière chaque semaine (système
plein-vide dans une armoire en SSPI). Afin d’évaluer l’impact de cette
nouvelle procédure, nous avons mené une étude prospective exposés
e non exposés (Chirurgie Viscérale et Orthopédique) de deux
groupes: non exposés ¼ prescription libre de l’ABP (ABPlibre) et
exposés ¼ utilisation de Kit par indication chirurgicale (ABPkit).
Résultats 420 patients ont été inclus. Les deux groupes sont
comparables pour le type de chirurgie (Orthopédie 33%, Chirurgie
Viscérale 66%), âge et sexe des patients, classes ASA et Altemeier et
durée d’intervention. Une ABP est prescrite dans 88% des cas avec
ABPlibre vs 90% des cas avec ABPkit, (p¼ns). Les principales
modalités de réalisation de l’ABP montrent une amélioration significative de la conformité de l’indication (93% vs 86%, p¼0.01), de la
conformité de la molécule choisie (88% vs 81%, p¼0.04). Le seul
paramètre non amélioré, ce qui correspond à un problème non ciblé
par les kits, est le délai entre l’administration de l’antibiotique et
l’incision chirurgicale (contraintes de fonctionnements d’équipes).
La mise en place des ABPkits a permis la suppression du stock
d’antibiotiques au bloc et une traçabilité des prescriptions par une
informatisation en temps réel intégrée dans le système d’information de l’hôpital. De 26 molécules pour l’ABP avant les kits, nous
sommes actuellement à 8 molécules. La conformité totale au référentiel passe de 75% à 87% dans le groupe ABPkit (p<0.01).
Discussion la mise en oeuvre des kits par indication chirurgicale a
permis d’améliorer la conformité globale aux référentiels. Cette
organisation pérenne depuis juin 2005 assure la traçabilité réglementaire des prescriptions antibiotiques, ainsi qu’une meilleure
gestion (péremption.).

Blood loss and transfusion rates remain high during total knee
arthroplasty (TKA), despite the availability of multiple strategies to
prevent or lessen the likelihood of these outcomes.
After having developed a database to assess the effectiveness of
blood salvage systems, we used a blood conserving plan based on
efficient and safe pharmacological agents in attempt to reduce the
homologous blood transfusions and post- operative anaemia in
primary TKA
Program The treatment plan comprise abandon of auto-transfusion
techniques (ABD, cell saver), elementary iron preoperatively,
erythrocytar induction using epoetin alpha when a deleterious post
operative anaemia is predicted and administration of tranexamic
acid, in except to contraindication. The postoperative analgesia was
multimodal. Anti-thrombosis prophylaxis was started with fondaparinux.
Data collected: haematocrit and haemoglobin levels on the
morning of day 1 and day 7, transfusions, calculation of total blood
losses throughout the post operative period of 7 days, Doppler
ultrasound of inferior limbs at postoperative day 8, complications
and costs
Résults A prospective analysis of 91 unilateral TKA performed in
using a blood transfusion-sparing plan (study group) was compared
to an historic group of unilateral TKAs operated in using post
operative salvage. None autologous blood predonation was utilised
in either group
In the study group, tranexamic acid (TA) and epoeitin alpha were
selectively administrated to reduce the blood losses and transfusion
requirements .In the historic group, none of these pharmacologic
agents was used and a post operative salvage was utilised. The total
blood loss was 1480 ml in the study group and 1800 ml in the cell
saver group throughout the perioperative period of 7 days.
The mean haemoglobin level was significantly superior in the
study group than in the control group at postoperative day 8 (11 g/
dl vs 10. 3 g/dl.)
In the study group, a sole patient aged of 89 years was blood
transfused after surgery made without tourniquet and TA because
of an arterial disease.
In the cell saver group, 41 of 44 patients received autologous
blood transfusion from cell saver and the mean re-infused blood
volume was 315 ml.
No patient having received epoetin preoperatively was blood
transfused.
The reduction of blood losses induced by the administration of
tranexamic acid was not different between a single bolus injection
and a continued administration of 24 h in our randomised controlled
clinical trial.
There were no thromboembolic or vascular complications in the
present series.
Discussion Recent improvement in surgical and anaesthetic procedures allow for performing routine unilateral TKA with a marginal
rate of blood transfusion when a blood transfusion sparing plan is
implemented (Couvret 2008)
Then, the potential risks of blood transfusion are avoided.
Tranexamic acid allows reducing immediate bleeding and transfusions requirements .By reducing the external blood loss, it makes

unnecessary the use and expense of a cell salvage drain as a blooddsparing strategy (Alvarez 2008)
In our study, like others, no complications thromboembolic
complications were documented among patients treated with
tranexamic acid. Because the big question in the prophylactic use of
tranexamic acid continues being its safety concerning the
thromboses events, we administrated fondaparinux in the post
operative period in all patients and made systematically ultrasonography to detect thrombosis.
Conclusion A blood transfusion sparing plan allows improving
quality of care by reducing the risks and costs of blood transfusion.
Despite the lack of blood transfusion, a satisfactory haemoglobin
level at postoperative day 7 can be achieved, when the blood sparing
transfusion plan is used by an experienced team treating patients
with a high mean BMI.
L’anémie postopératoire est une complication reconnue des patients
opérés de PTG primaires et la transfusion sanguine est utilisée pour
corriger la perte de sang. Le risque de transfusion varie de 4 à 72%
selon les centres. Différentes techniques existent pour tenter de
diminuer le recours à une transfusion homologue. Après avoir évalué
nos pratiques, nous avons mis en place un plan stratégique d’épargne
sanguine reposant sur des mesures associant efficacité et sécurité,
notre objectif étant d’obtenir un taux de transfusion homologue
faible associé à une réduction des anémies postopératoires.
Programme Les mesures mises en place sont:
Discussion La mise en œuvre d’un plan d’épargne transfusionnel
global pour les patients opérés de PTG primaires permet de
marginaliser le taux de transfusion (Couvret 2008) y compris chez
les patients ayant reçu de l’EPO (Hourlier 2009).
Il permet d’éviter les risques liés à la transfusion sanguine.
L’acide tranéxamique a un rôle essentiel pour:
< diminuer la déperdition sanguine des premières heures post
opératoires;
< marginaliser les besoins transfusionnels qui impactent la
durée d’hospitalisation (Weber 2005);
< réduire la sévérité de l’anémie post opératoire qui influe sur la
qualité du recouvrement fonctionnel (Conlon 2007).
Ce traitement peu onéreux rend inopérantes les techniques
d’autotransfusion (Alvarez 2008) qui sont coûteuses et source de
gaspillage.
Conclusion L’utilisation d’un plan d’épargne transfusionnel global
pour les PTG permet de rendre marginal le recours à une transfusion
homologue, d’en éviter les risques et de réduire la consommation de
produits sanguins alors que les besoins globaux augmentent et que
l’offre se réduit. Malgré l’absence de transfusion, l’anémie postopératoire reste modérée lorsque le plan est appliqué par une équipe
expérimentée traitant des patients ayant un BMI élevé.

< l’abandon des techniques d’autotransfusion (TAP, cell-saver
ou RPO);
< un apport ferrique préopératoire systématique;
< un traitement inducteur érythrocytaire par epoietin alpha
(EPO) en cas de calcul prédictif d’anémie postopératoire
délétère;
< l’utilisation systématique d’acide tranéxamique (AT) sauf
contre indication.
L’analgésie postopératoire est multimodale. La thromboprophyllaxie est confiée au Fundaparinux.
Les éléments d’évaluation comportent:
< des hémogrammes pré et post opératoires réalisés à des
intervalles réguliers (J1 et J7);
< le recueil des données transfusionnelles;
< le calcul des pertes sanguines sur la période périopératoire au
7e jour;
< un examen doppler veineux des membres inférieurs au 7e
jour;
< le recueil et l’analyse des complications postopératoires;
< l’analyse des coûts.
Resultats Cette stratégie est mise en place sur une série consécutive
de 91 patients (âge moyen 74 ans, BMI 30) opérés en 2008 de PTG
unilatérale. Elle a été comparée à une série de patients traités en 2005
avec un récupérateur de sang.
Les pertes de sang total sont en moyenne de 1,5 L dans la série
d’étude versus 1,8 L dans la série de 2005. L’hémoglobinémie
moyenne au 7e jour post opératoire est de 11 g/dl versus 10,3 gr/dl.
Dans la série d’étude, un seul patient a été transfusé. Il s’agissait
d’un patient âgé de 89 ans, opéré sans garrot et sans AT en raison
d’un terrain artériel.
Aucun patient ayant reçu de l’EPO en préopératoire n’a été
transfusé. Aucun n’a eu un taux d’hémoglobinémie inférieur à 10 gr/
dl en postopératoire.
La réduction des pertes sanguines induite par l’administration
d’acide tranéxamique n’a pas été significativement différente sur une
analyse faite de façon randomisée et contrôlée en double aveugle
concernant un protocole d’injection unique ou continu sur 24
heures).
Il n’y a pas eu de complications thromboembolique ou cardiovasculaire.

Background and objectives For Morbidity Monitoring Staff (MMS)
meetings to be efficient, they need to include all patient files
concerned with medical failures or logistics failures. The major
difficulty is selecting all relevant files. Our objective was to develop a
computer application that would select the files that need to be
discussed in order to identify medical failures and logistics failures
that need remedying.
Programme The computer application for automatic file selection is
based on a comparison of the actual length of hospital stay with the
predicted length of stay for a given disease and/or surgical procedure.
Every 2 months, all selected files are reviewed to determine which
files are incomplete, which relate to medical failures, and which to
logistics failures.
Results During the 30-month period from January 2006 to April
2009, 8540 children underwent surgery in our department. A total of
1364 (16%) medical files were automatically selected for review by
MMS. A total of 51 cases (0.6%) of medical failure and 16 cases
(0.19%) of logistics failure were identified and analysed. Three
improvement actions ensued: (i) the development of a specific
multidisciplinary medical file for ASA III patients, (ii) routine
bacteriological analysis in cases of appendectomy in order to fight
antibiotic-resistant infections better, (iii) changes in the organisation
of day-care surgery (phone-call the day before and after surgery,
changes in the timetable.)
Conclusion The automated procedure for the selection of medical
files for review by MMS helped us identify medical failures (0.6%

rate) and initiate specific actions in order to improve health care
delivery in our department. Our tool may be useful for MMS in all
cases of scheduled hospitalisation or elective surgery.
Introduction l’amélioration des pratiques professionnelles repose
entre autre sur la réalisation au sein des équipes médicales et paramédicales de RMM.
Toutefois, la sélection des dossiers à analyser dépend le plus
souvent d’une évaluation individuelle de la survenue d’un évènement porteur de risque ou d’une complication et n’est pas toujours
exhaustive.
Afin de pallier à cette situation, nous avons développé un système
automatique de sélection des dossiers à analyser, basé sur la durée du
séjour hospitalier et le caractère complet ou nom du dossier (présence de résumé d’activité médicale et de compte rendu opératoire).
Ce mode de sélection nous permettant d’extraire les dossiers
incomplets et de les finaliser, mais aussi de mettre en exergue les
complications médicales et les problèmes organisationnels.
Matériel et méthodes Une application informatique développée sur
le logiciel ACCESS (microsoft*) permet, pour chaque patient opéré
dans le service, de comparer la durée de l’hospitalisation effectuée à
la durée prévisible de l’hospitalisation pour la pathologie et le geste
chirurgical concerné. Ainsi, tout patient dont la durée dépasse la
durée prévisible est sélectionné et analysé lors de la revue de morbimortalité mensuelle. L’analyse pluridisciplinaire de tous les dossiers
sélectionnés permettant de distinguer les problèmes organisationnels
des complications médicales qui sont secondairement discutées.
Résultats Sur une période de 30 mois (janvier 2006 e mars 2009)
8540 enfants ont été pris en charge dans le service. Sur l’ensemble
des dossiers, 1364 dossiers (16%) ont été automatiquement sélectionnés et analysés en RMM. 51 dossiers (0,6%) ont été extraits pour
des complications médicales et 16 pour des problèmes organisationnels (0,19%).
L’analyse rétrospective des dossiers a permis de générer plusieurs
démarches qualités pour la prise en charge des patients telles que:
< La constitution d’un dossier patient multidisciplinaire pré et
post-opératoire pour les patients ASA III.
< La réalisation d’une analyse bactériologique en cas d’appendicite aigue afin de faire face à l’apparition de germe résistants
aux antibiotiques usuels.
< La modification de l’organisation de prise en charge ambulatoire des patients (appels téléphoniques la veille et le
lendemain de l’intervention, horaire d’admission.).
Conclusion L’utilisation d’un outil informatique de sélection automatique des dossiers analysés en RMM nous a permis de révéler un
taux de complications médicales de 0,6% et de générer plusieurs
démarches visant à améliorer la prise en charge des patients. Cet
outil nous semble aisément applicable dans des domaines de prise en
charge programmée des patients.

Background, objectives Episiotomy rates are a national concern and
a major source of dissatisfaction among healthcare users. National
good practice guidelines (GPG) issued in 2005 recommended that

the episiotomy rate should be reduced to less than 30% (http://
www.cngof.asso.fr). Nonetheless, some countries have much
lower rates and the figure of 20% should be reachable without
difficulty. The Clermont-Ferrand university hospital centre (UHC)
has two maternity units. In 2004, maternity ward 1 had an episiotomy rate of 29.25% and maternity ward 2, 39.27%. The principal objective of the programme described herein was to reduce
these rates.
Programme An indicator approach was chosen with annual followup of the overall episiotomy rate, to begin in 2006. In 2005, ward 1
had an episiotomy rate of 22.35% and ward 2 of 33.62%.
A retrospective targeted clinical audit was also performed on 100
case files covering the period from 1 July though 1 December, 2005.
The indication for the episiotomy was reported in 48% of cases, the
name of the person who performed it was available in 87%, and the
name of the person who did the repair in 86%. The type of thread
used was available in only 68% of cases. A complete report of the
repair was included in 64% of the files. No file included a pain
assessment. The use of anaesthesia during the repair was noted in
90% of cases and all cases with suture complications included a
medical observation.
Steps were taken and activities conducted to reduce the rate
between 2006 and 2008. They consisted of the annual dissemination
of the episiotomy rate compared with the regional rate to all
midwife managers (for further dissemination) and to physicians.
The Auvergne Perinatal Health Network (RSPA) also relays this
dissemination each year. Awareness campaigns were conducted for
healthcare professionals, during inhouse departmental meetings or
regional conferences, over a 3-year period. A structured computer file
was developed and made available to all; it includes items related to
episiotomies and allows direct viewing of all national guidelines
including on those on episiotomies. Finally, training was provided
for interested volunteer midwives and physicians in delivery positions other than dorsal decubitus.
Clinical impact Over this period the overall episiotomy rate has
fallen markedly: in 2008, it was 19.34% in ward 1 and 17.93% in
ward 2. A second targeted clinical audit was performed on 85 files of
women who gave birth in one of our two maternity wards between
1 July and 1 December, 2007. It showed an improvement in all the
criteria considered in the audit except for the assessment of pain
during the repair. Since 2006, annual comparisons have been
conducted with national rates, via the Audipog registry, and with
regional rates via the RSPA.
Discussion, conclusion The working group decided at the end of
2009 to close this practice evaluation process, but to continue to
provide the teams with their episiotomy rates together with the
regional and national means, via Audipog (http://www.audipog.
assets). The rates reached suggest that there is no longer substantial
room for improvement, especially considering the large number of
problem pregnancies in our reference centre. Participation in this
process and its positive impact on practices were aided by the RSPA
policy that adapted this national objective to local conditions and
promoted it. The presence of physician-leaders, including the
department head, who had also participated in developing the 2005
GPG, was also a key element.

santé. La recommandation nationale (RPC) de 2005 recommandait
d’avoir moins de 30% de taux d’épisiotomies (http://www.cngof.
asso.fr). Cependant, certains pays ont des taux beaucoup plus bas et
le chiffre de 20% pourrait être atteint sans difficulté. Notre
établissement universitaire (CHU) dispose de deux maternités En
2004, la Maternité 1 avait un taux d’épisiotomies de 29,25% et la
Maternité 2 de 39,27%. L’objectif principal de cette démarche était la
réduction du taux d’épisiotomies.
