PHOTOGRAPH AND VIDEO RELEASE FORM
I grant permission to the YM-YWHA to use photographs and/or videos taken of me for use on YM-YWHA web sites
or other electronic form or printed media, and to offer the photographs and/or videos for use or distribution to
other YM-YWHA departments, without notifying me.
I hereby waive any right to inspect or approve the photographs, videos, or electronic matter that may be used in
conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right
to royalties or other compensation arising from or related to the use of the photographs and/or videos.
I hereby agree to release and hold harmless the YM-YWHA via electronic or media, from and against any claims,
damages or liability arising from or related to the use of the photographs and/or videos, including but not limited
to any re-use, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or
otherwise, that may occur or be produced in production of the finished product.
I am 18 years of age or older and I am competent to contract in my own name OR I am the guardian or parent of
the minor in the photographs and/or videos. I have read this release before signing below, and I fully understand
the contents, meaning and impact of this release.
I understand that I am free to address any specific questions regarding this release by submitting those questions
in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable
acceptance of the terms of this release.

moyen de diffusion électronique ou dans les médias, et d’offrir ces photos ou vidéos pour leur utilisation et
distribution par d’autres départements du YM-YWHA, sans autre préavis.

altération, illusion d’optique ou utilisation dans la composition de formulaires, de façon intentionnelle ou non,
qui pourraient survenir dans la production du produit final.

mineur figurant dans la photo. J’ai pris connaissance de cette décharge avant de la signer ci-dessous, et j’ai
compris tout son contenu ainsi que la signification et l’impact de cette décharge.
Je comprend que je suis libre de poser toutes questions pertinentes concernant cette décharge en les adressant
par écrit avant de la signer et j’accepte que si je ne le fais pas, cela voudra dire que je confirme mon acceptation
des termes de cette décharge.
