CIOMR Marcia Quinn
Dear Colleagues
Chers Collègues,
Well another Congress will soon be upon us and with
Viterbo, the end of the Canadian Presidency. It has been a
fascinating two years filled with changes and challenges for
all of our Armed Forces and especially for our Reserves as
Nations are change how they employ and deploy their
Reservists.
We now increasingly find that most are
changing from a mobilization-based
capability, which has been the traditional
(historical) reserve mandate for many, to
one which is more based upon an
augmentation and sustainment model.
Though it can be said that the pace of
change has been happening over the
past ten years, it seems that it has
accelerated, especially within the last two
years. This excellerated pace, in what for
many is a role change, has challenged
many nations as they try to meet their
goals and responsibilities with their
forces in being.
This is why it is especially important to network amongst
ourselves and share our lessons learned. We need to share
much of this information with our active forces as we all try
to meet rising demands. Only by devising and applying a

common/standardized plan of action can we hope to
succeed in today’s high tempo environment.
The common challenge of augmenting and sustaining our
active forces, and yet coping with the demands of our civilian
workplace are struggles more and more of our members are
experiencing.
It is a challenge we must overcome and it is during
conferences such as the Viterbo Congress, which offer a
great venue for these discussions. From the skills workshops,
such as the neuro workshop of last year and ACLS for
advanced providers to be given this summer to the panel
discussions on homeland defence issues, all our work
continues to shine an important light onto the issues and
challenges we face. It also exhibits the ingenuity and
professionalism of a group who are dedicated to the care of
our military personnel and those others who may, from time
to time, come under our care.
I look forward to seeing you in Viterbo and continuing the
development of our action plan for the future. Safe journey
to all of you.

Telemedicine in M.A.S.H. By Leo Kronberger, MD (AUT)
Telemedicine means medicine at a distance and combines the use of telecommunications, computer technologies, and
informatics – the applied science of collecting, storing, and retrieving data to support informed decision making
In the Future even after continuous access to the Internet would be provided to all Medical Field Units, there may never be
enough bandwidth available to the primary care providers. Their medical information needs are always secondary to the
military optional information needs and in times of conflict, such bandwidth may be turned off to reduce the electronic
emissions signature. This simple fact leads to a requirement for continuous refinement and enhancement of the caching
strategy under the assumption that patrons can only expected to have intermittent access to the Internet.
Nevertheless personal care and communication is important in the future as it was in the past and the present. From “Future The Aventis magazine” it should be cited In years gone by, I was often asked, “Will the computer replace the doctor?” My rejoinder, still apt
today, is that any doctor who can be replaced by a computer deserves to be.
New Telemetric Device for Central Patient Monitoring by Capt Thomas SYBURRA (CHE)
BACKGROUND
Pulse oximetry is an established technology for monitoring of patients. At our institution we developed a new device
(“Auricall”) that makes this technology available for mobile in- and outpatients. It is a small device worn continuously on the
ear (Bluetooth). The data are transferred by mobile telephone network (GSM/GPRS) and can be easily accessed by the
specialist in order to optimize therapy. In case of an emergency it is sent automatically to the doctor in charge or to a rescue
center in case of outpatients.

Spring/Printemps 2006
Aim of the study is to demonstrate the reliability and practicability of the new telemetric device in patients presenting
desaturations and/or arrhythmia. In addition, a potential health care costs reduction achieved by possible earlier discharge
from the ICU, from the hospital and from the rehabilitation centres was analyzed. The system should allow fewer caregivers to
treat a greater number of patients with the availability of central surveillance monitoring.
CONCLUSIONS
The results demonstrate that “Auricall” is a reliable device for monitoring of SpO2 and heart rate. An ECG monitoring is
actually being integrated. The fact that it can be done continuously and independently of the patient’s location, including
foreign countries, has major advantages. It offers great benefits for therapy as it early recognize desaturations and arrhythmias,
the effect of pharmaceuticals over time, and the development of the values during typical life situations independently of the
patient’s location and activities.
Clinical Research in Defense Medicine byGunter H. Ruetter, MD, PhD, COL (MC) GE JMS AFR
Asymmetric harassment and attacks require new approaches also for defense medicine. CBRN Defense needs to develop
medical measures for both prevention and mitigation of CBRN affection to the unprepared human body.
Unbalanced measures with poor efficacy and/or severe side effects have shown that clinical evaluation is mandatory in order
to provide effective and efficient measures for prevention and/or mitigation of CBRN affection.
Clinical research is a discipline which usually focuses on new therapeutic drugs, medical devices or diagnostics,
which need to be evaluated in controlled clinical trials before they may be approved for regular medical use. These
clinical studies on volunteers use to be carried out according to GCP/ICH guidelines which are globally accepted by
the responsible agencies. The fields of application comprise the whole spectrum of health care.
As CBRN Defense is required to evaluate all medical measures for best medical management, utilization of specific
expertise in clinical research is mandatory.
Bringing technology to education: the impact of human patient simulator on training
by Capitaine Julien Clément MD, Canadian Forces

New territorial organisation of french military health service.
Colonel ( r) Jean-Pierre CAPEL
Organization and operating of regional departement of the military health service in France was, until now, founded on
specific departements for Army, Navy and Air Force, although the service was unified since many years.
Since july 1st , 2005 and a new organization, 6 regional departments are supervising medical, dental and veterinary services
and assuming the medical support of all the defence units.
Organization, allocations specialy in matter of professional and reserve training, military specific medicine, prophylaxy and
hygiene or evaluation medicine are still remended and a new fonction is given to the « liaison officer of the military health
service » between the regional commandment (Army-Navy-Air Force) and the regional department of the military health
service.

An official recognition
Cher Colleagues,
Chers collègues,
It has been an eventful time for us with a landmark decision
reached by the COMEDS on 29 November 2005. During
their Plenary session held at NATO HQ, Brussels, 28-30
Nov 05, the CIOMR was unanimously accepted as an official
observer of all COMEDS meetings.
As with the other Observers, it is anticipated that we will be
expected to submit liaison reports at the annual Plenary
session. This is done by the Defence Group on Proliferation
(DGP) given by the IMS representative; The MC Medical
Standardization Board (MCMedSB), the Joint Civil-Military
Committee (JMC), the Human Factors of
Medicine-Research and Technology Organization (RTOHFM) and the Weapons of Mass Destruction Committee
who all have representatives giving liaison reports.
These reports will focus on our new format of scientific
symposia and military medical skills workshops, which have
piqued the interest of the various Surgeons General. I have
asked our Sec Gen to have our webmaster post a copy of our
presentation to the COMEDS on our website for your
information.
COMEDS felt it necessary to put certain caveats in place,
since we represent more than just NATO members. These
are:
1) Nominations for members of any of the COMEDS
Working Groups must always go through the office of
their national Surgeon General; and
2) Because we represent more than just NATO countries
(analogous to the EU Military representative who is also
an observer), we cannot attend "NATO Only" sessions
of the COMEDS meetings.
As a matter of interest, there are a couple of new acronyms
that may be of use to our members. After a major
restructuring of the NATO military commands begins as the
Headquarters of the Supreme Allied Commander,
Atlantic was abolished and a new command, Allied
Command Transformation (ACT), was established in
Norfolk, Virginia, U.S. and the Supreme Headquarters Allied
Powers Europe (SHAPE) became Allied Command
Operations (ACO). ACT is responsible for driving
transformation (future capabilities) in NATO, whilst ACO is
responsible for current operations. We will get ourselves on
the mailing list of the ACT Quarterly Newsletter in the next
short while.
I feel this is the beginning of a transformational partnership
which will see our work brought into more prominence. We
have an impressive product (or to use the latest term:
deliverable) and this can be used by all our members
Bulletin CIOMR Newsletter
National de leur médecin général national ;

Neurosurgical Head Injury Practical
Workshop - Ghent, 02 Aug 2005
Atelier pratique sur les traumismes
crâniocérébraux – Gand, 02 Aou 2005
The format of the CIOMR Summer Congress was changed
this year with the introduction of a neurosurgical head injury
workshop. This was led by the UK delegation with the
addition of a lecturer from Denmark.
The Defence Medical Services require surgeons to be
competent to operate on any area of the body, true “General
Surgeons”. Such surgical capability is decreasing rapidly with
civilian training directed towards specialisation. To address
operational requirements, the UK has devised a military
surgical training programme, of which the neurosurgical
workshop is part. Because the workshop has been so
successful in the UK and there being no other similar
courses to our knowledge in NATO, it was suggested and
later accepted by the committee of CIOMR that one should
be conducted at the Summer Congress.
The workshop took place at the Veterinary School of the
University on 2 Aug 05. It consisted of lectures on blunt and
penetrating head injuries and practical workshops on burr
holes, craniectomies, depressed skull fractures and gunshot
wounds of the head on the isolated pig’s head model.
Photographs are attached.
The workshop was deemed to be a great success. Delegates
came from 9 countries and 11 different disciplines. The
appraisals at the end of the day were very encouraging. The
structure of the course was given an ‘Excellent’ rating by
82% and the practical skill stations 81%.General comments
were : “Excellent course, well organised and brilliant
presentation; I hope we import this into our own country”
We were very grateful to the Royal Belgian Medical Military
Reserves Association as well as the University Faculty of
Veterinary Medicine without whose support the course
would not have been possible.

CIOMR Viterbo, Italy : Continuing Medical Education:
Resuscitation Workshop, Monday 3 July 2006
CIOMR 2006, Viterbo, Italie - Formation médicale continue
Atelier sur la réanimation, lundi le 3 juillet 2006
1. Introduction
1. Introduction
Following the successful British led Neurosurgical Skills
workshop in Ghent 2005, a Continuing Medical Education
(CME) event occurring in conjunction with the annual
summer CIOMR summer meeting scheduled for Viterbo
2006 is planned. It will allow clinicians to reaffirm some
military medicine skills, focusing on the relevant military
topic: Resuscitations.
Rome 2006. Au cours de cette activité, les cliniciens
pourront actualiser certaines de leurs compétences en
médecine militaire en traitant principalement du sujet
militaire susmentionné : la réanimation.

Professor, Dept. of Emergency Medicine, University of
Alberta, Medical Director Edmonton Heartsafe
Program, Medical Director Edmonton Emergency
Response Department
Maj. Roger Scott RN, EMT-P, Master's candidate Nurse
Practitioner Program University of Alberta
3. Course Outline
The Advanced Cardiac Life Support - Experienced Provider
(ACLS-EP) course is a Heart and Stroke Foundation
recognized and developed course. It was designed as an
alternate way of recertifying practioners in ACLS. It has
been run in North America for over 4 years after being
developed following the ACLS guidelines of 2000 being
introduced. It has been well received as a viable
recertification program and is delivered to various health care
providers, including pre-hospital care providers.
Bulletin CIOMR Newsletter
Edmonton;
- Le Maj Roger Scott, IA, EMT-P, maîtrise du programme
de formation des infirmiers/infirmières
praticiens/praticiennes de l'Université de l'Alberta.

Spring/Printemps 2006
We will modify this course to meet the military theme. The
course can effectively be run in one day in the following
sequence:
a.
2 hr ACLS refresher and review of new 2006
Emergency Cardiac Care Guidelines to be done thru
small facilitated group discussions on the current
ACLS algorithms through case based discussions);
and
b. 1 hr sessions known as the Experienced Provider
component where learners are lead through up to 4
cases, each focusing on the different types of Special
Resuscitations.
This second component focuses on immediate management
of particular patient presentations and the periarrest state,
immediate management issues to prevent the development
of cardiac arrest and the management of cardiac arrest
should it occur. The cases can vary according to the student
population and interest. The faculty has chosen the
following scenarios:
i.
ii.
iii.
iv.
Trauma/Environmental (Lightening/Electrical
Injury);
Asthma;
Toxicology;
Acute Coronary Syndromes.
The course will be held on July 3rd at NCM academy of the
Army, Viterbo, Italy. We anticipate their will be a minimal
fee for lunch. Amount to be confirm at a later date. There
are NO other associated cost to attend this workshop.
4. Target Audience
This continuing medical education seminar is targeted at the
Physician, nurse, mid level provider (paramedic, nurse
practitioner, physician assistant) who will be involved in the
resuscitation of patients. Enrolment is limited to 15 people
so apply soon. All candidates should have experience with
current ACLS guidelines ( i.e. participation in recent ACLS
provider course < 5 years).

Liaison Committee on Resuscitation (ILCOR)
(discussions animées en petits groupes concernant les
algorithmes de la technique à l' aide de cas);
fournisseur d' expérience pendant lesquelles on
i.
ii.
iii.
iv.
Trauma/environnement (blessure liée à la
foudre et à l'électricité)
Asthme
Toxicologie
Syndromes coronariens aigus
