Inc. was appointed Receiver tothe assets of LevinoffColbex, S.E.C., pursuant to Section 243 ofthe Bankruptcy
andInsolvency Act("BIA").
Le sequestre vous avise que, contormernent au Reglement
sur Ie Programme de protection des salaries, vous etes
tenu de deposer une preuve de reclamation selon les
dispositions de la Loisurla fail/ite etl'insolvabilite (Canada)
pour tout salaire dO en date de lamise sous sequestre.
Une preuve de reclamation a etecompletee envotre
nom par la compagnie, pourlessalaires dus,

Earner Protection Program Regulations, you are required
to file a proof ofclaim in accordance with the Bankruptcy
andInsolvency Act(Canada) for wages outstanding asat
the date ofthe receivership. A proof of claim form has
been filed on your behalf by the Company, with respect
to wages, vacation payand severance pay(see
attached table). If you do not agree with the amounts
filed, a blank proofof claim is attached herewith for
you to complete (include explanations of the change)

payes par Ie sequestra, vous pouvez deposer une
demande aupres duministre qui decidera devotre
admissibilite selon la loi en vigueur.
Should you believe that you have unpaid wages that are
eligible for payment under the Wage Earner Protection
Program which have not been paid bythe Receiver, you
may file an application with the Minister and eligibility will
be determined bythe Minister inaccordance with the
legislation.
You will also find enclosed herewith an application form for
the Wage Earner Protection Program which you must
submit to the Government.

Reglement sur Ie Programme de Protection des salaries, Ie
sequestra a produit ettransmis les renseignements
necessaires au Ministre Ie 16juillet 2012, soit dans un delai
superieur a45 jours car nous n'avons pu obtenir I'information
pertinente plus tot.
Please note that, pursuant to sections 9 and 15(2) of the
Wage Earner Protection Program Regulations, the
Receiver has produced and transmitted the necessary
information tothe Ministry on July 16, 2012, that is to say,
more than 45 days after the date of the receivership aswe
did not receive the related information sooner.
Vous devez deposer votre reclamation envertu du PPS
aupres de RHDSC/Service Canada avant Ie25 juillet
2012.
You mustfile your WEPP claim with HRSDC/Service
Canada before July 25,2012.
