Introduction Carbon monoxide (CO) is a toxic, odourless, colourless and tasteless gas. It is formed by the incomplete combustion of organic material at high temperatures, with an insufficient oxygen supply. When inhaled, CO displaces oxygen in the bloodstream to form carboxyhaemoglobin (COHb). This starves body tissue of oxygen restricting the body’s performance and causing health implications. CO can remain in the bloodstream for up to 24 hours, depending on factors such as physical activity, sex, and inhalation intensity. The half-­‐life is about 5 hours. CO (ppm), carboxyhaemoglobin (%COHb) and foetal carboxyhaemoglobin (%fCOHb) correlation: Breath carbon monoxide is measured in parts per million (ppm) and blood carboxyhaemoglobin is measured in percentages (%COHb and %fCOHb). The three are compatible and convertible: the ppm CO reading relates to gas in the lungs and on the breath (i.e. the amount of poisonous CO that has been inhaled), and the %COHb and %fCOHb readings relate to the percentage of vital oxygen that has been replaced in the bloodstream. The piCO™ Smokerlyzer® displays both CO ppm and %COHb, the piCObaby™ Smokerlyzer® displays both CO ppm,%FCOHb and %COHb and the Micro+™ Smokerlyzer® displays CO ppm, %COHb and %FCOHb. However all devices only directly measure CO ppm, the %COHb and %fCOHb is a calculation based on clinical evidence. Clinical research has demonstrated that a useful relationship between carbon monoxide and carboxyhaemoglobin is obtained after a short period of breath-­‐
holding. The cut-­‐off point between smoker and non-­‐smoker differs based on local regulation, Bedfont have created cut-­‐off levels as a guide only, see ‘Traffic Light Pre-­‐set Cut off Levels’, these can be altered in-­‐line with your local regulations from the settings screen. Taking a breath test 1. Attach a breath sampling D-­‐piece and new mouthpiece. 2. Turn on the monitor by pressing the power button once. 3. Press ‘Breath Test’ symbol on screen, 4. . . 5. Inhale and hold breath for the pre-­‐set 15 second countdown. 6. A beep will sound during the last three seconds of the countdown. 7. Blow slowly into mouthpiece, aiming to empty lungs completely. 8. The ppm and equivalent %COHb and/or %fCOHb levels will rise and hold on-­‐screen. 2 9. On the piCO™ and piCObaby™, when the test is finished of the screen. will appear at the bottom 10. On the Micro+™, when the test is finished screen. will appear at the bottom of the 11. If a high reading has been recorded, you can mute the sound by pressing . 12. To repeat breath test, press once to return to the home screen and repeat steps 3-­‐8. 13. To save the reading (Micro+™ only) press and select the relevant patient profile. 14. Remove the D-­‐piece between tests to purge sensor with fresh air. 15. To switch off, press and hold the power button for 3 seconds, unit will also power off after 2 minutes of inactivity to save power. Conducting an Ambient Test – Micro+™ only The environmental air can be tested to check for CO in the environment using the Micro+™ Smokerlyzer. This is particularly useful in the event of a patient reporting a non-­‐smoker status but providing a breath reading of that of a smoker suggesting they have inhaled environmental CO. This is carried out by pressing on the screen; the Micro+™ will then carry out real-­‐time monitoring of the environmental air. To stop the test, press . Bedfont do not recommend that the environmental test alone is used as a measure of passive smoking as the very high dispersiveness of CO at the levels involved with smoking can lead to false negative readings. Reviewing history – Micro+™ only The Micro+™ will record every reading taken in its history up to 150 readings, to access this press and then the history will then be shown. Changing date and time – Micro+™ only Press and then you can then select either d-­‐m-­‐y or m-­‐d-­‐y for the date format and 12h or 24h for the time format. Dates and times are then adjusted by selecting the number you wish to change and pressing or to increase and decrease. Mouthpieces should be replaced after every use. 2. Hands should be washed regularly in accordance with infection control practice. 3. Please do not attempt to modify the equipment in any way or use accessories not specified by the manufacturer. Any attempt to do so will invalidate the warranty and may compromise the safety of the device. 4. Bedfont will make available upon request service training to appropriately qualified persons. 5. Holding the reset button down for 30 seconds will perform complete device reset, this will clear any saved data and revert all settings to the factory defaults. After performing a reset the device will need to have the date/time set and be calibrated before it can be used. Cleaning 1. The Smokerlyzer® products are moulded with SteriTouch® technology for optimum infection control and bacterial efficacy. However Bedfont recommend wiping the instrument and D-­‐
piece external surfaces with a product specifically developed for this purpose. Bedfont provides instrument cleaning wipes. The D-­‐Piece cannot be sterilised. 2. NEVER use alcohol or cleaning agents containing alcohol or other organic solvents as long term exposure to these vapours will damage the CO sensor inside. 3. Under no circumstances should the instrument be immersed in liquid or splashed with liquid. 8 Routine Maintenance 1. Replace batteries when indicated by the empty symbol . 2. Bedfont recommend removal of the batteries when the device is not used for prolonged periods of time to prevent leakage. 3. Replace breath sampling D-­‐piece every 30 days or if visibly soiled or contaminated. The piCO™, piCObaby™ and Micro+™ Smokerlyzer® products will give a reminder during start-­‐up when the D-­‐piece should be replaced, see ‘Change D-­‐Piece’ symbol. 4. The sensor should be replaced every 5 years, 60 days prior to the sensor change ‘Countdown to sensor change’ symbol will be shown with the date on which the sensor should be changed. This can be ignored by pressing until the date at which the sensor should be changed arrives, at this point the ‘Change Sensor’ symbol will be shown. Change sensor if trained to by an approved Bedfont engineer or send to Bedfont or the local representative. 5. Additional technical information can be made available on request; please contact Bedfont or its distributor. Troubleshooting The unit fails to turn on: If the unit fails to turn on, replace the batteries. Ensure that the batteries are inserted the correct way around, matching the symbols moulded into the plastic. High readings recorded for reported non-­‐smoker: If a breath test on a non-­‐smoker gives an indication of CO being present, it may indicate the following: 1. A high ambient level of CO (possibly due to a faulty combustion appliance or exhaust fumes). 2. The effects of passive smoking. 3. The device is contaminated by alcohol or another organic substance. 4. The patient may have a very high level of hydrogen on their breath, which can be generated in the human digestive system. 5. The sensor may have drifted out of specification (see below). The sensor has drifted out of specification: Smokerlyzer® products are calibrated before leaving Bedfont, however we recommend if you suspect the instrument is reading incorrectly, try the test again with another device if available to get a comparison. Alternatively you check the function using Bedfont check gas or send back to Bedfont. The check gas required is Bedfont 20ppm carbon monoxide in air, procedure below. 1. Ensure the fine control valve is in the off position. 2. Screw the fine control valve and flow indicator assembly to the gas can. This is best done by screwing the gas can into the valve. 3. Press ‘Breath Test’ symbol on screen, or
, to start a breath test. 4. When prompted by the ‘Exhale’ symbol, or

If the final displayed value is less than 15ppm or higher than 25ppm, stop the test and perform a calibration or send back to Bedfont for a free of charge calibration, following the instructions below. Calibration procedure: 1. Turn on the monitor by pressing the power button once. 2. Press . 3. Start the calibration process, pressing the proceed. symbol 5 times and then once to 4. COdata+™ Software – connecting to the PC Place one end of the connection lead into the USB socket on the top of the Smokerlyzer® product, connecting the other end to the USB port on the PC. Before starting the software, ensure that the Smokerlyzer® product is connected to the PC and switched on. Double click the COdata+™ icon on the PC to start the programme. Refer to the supplied documentation for how to operate COdata+™. 10 Returns Procedure If your equipment requires servicing, please contact your local Bedfont Customer Service Department, distributor or supplier before returning any goods. 1. When you have supplied the Customer Repairs Department with the monitor serial number and description of the fault, you will be issued with a returns/ ticket number. Please state the returns/ticket number on the outside of the box when returning the monitor, and ensure that your full details, telephone, fax numbers and return address are clearly stated. 2. The product must also be decontaminated before it is returned according to your local regulations. Bedfont can provide you with a decontamination certificate to complete, which also needs to be attached to the outside of the box. Failure to do so will result in the product being subject to Bedfont decontamination procedure and will delay your service/repair. 3. Bedfont advise that you use a courier service when returning monitors. This enables you to insure goods for loss or damage in transit. When your goods are received, you will be sent an email stating so. 4. If the device has been returned for repair it will then be examined and you will be sent an ‘Engineer’s Report’ and a quotation for the repair, which will include an authorisation form. Complete the authorisation form, and ensure that you include the ‘Official Purchase Order Number’. Please contact the Customer Repairs Department if you are unable to supply an ‘Official Purchase Order Number’. 5. If your monitor is still in warranty and the fault is covered by warranty, see ‘Warranty’ section of this manual, Bedfont will repair it and return it to you with an ‘Engineer’s Report’, free of charge. 6. If you choose not to proceed with the repair, a handling fee will be charged. Ensure that you return the completed authorisation form with an ‘Official Purchase Order Number’. 7. The equipment will be returned to you as soon as Bedfont has received all relevant paperwork. A carriage fee will be charged if the monitor is no longer in warranty. These warranties are automatically invalidated if the products are repaired; altered, void labels removed or otherwise tampered with by unauthorised personnel, or have been subject to misuse, neglect or accident. Never dispose of any electronic instrument or batteries in domestic waste. Introduction Le monoxyde de carbone est un gaz toxique, inodore, incolore et sans saveur. Il résulte de la combustion incomplète, à très hautes températures, de matières organiques combinées à une alimentation en oxygène insuffisante. Quand il est inhalé, le CO prend la place de l’Oxygène sur l’Hémoglobine formant le complexe Carboxyhémoglobine (HbCO). Ceci a pour effet de limiter l’oxygénation des tissus qui est vital pour le bon fonctionnement de l’organisme. Le CO peut rester dans le sang pendant plus de 24h en fonction de l’activité physique, du sexe de la personne et de l’intensité de l’inhalation. La demi-­‐vie du CO est de l’ordre de 5h. Corrélation entre le CO (ppm), la carboxyhémoglobine (%COHb) et la carboxyhémoglobine fœtale (%fCOHb): Le monoxyde de carbone expiré se mesure en partie par million (ppm CO) et la carboxyhémoglobine dans le sang en pourcentage (%COHb et %fCOHb). Les trois sont compatibles et convertibles, le CO se rapportant à la quantité de gaz dans les poumons/les voies respiratoires (correspondant à la quantité de CO nocif qui a été inhalée) et la COHb et fCOHb à la quantité de gaz présente dans le sang. L’analyseur piCO™ Smokerlyzer® affiche la concentration en CO ainsi que %COHb. Le piCObaby™ Smokerlyzer® et le Micro+™ Smokerlyzer® affichent en plus %fCOHb. Cependant, tous ces appareils ne mesurent que directement ppm CO ; la %COHb et %fCOHb est un calcul basé sur des données cliniques. Des études cliniques ont montré qu’une relation existe entre le monoxyde de carbone et la carboxyhémoglobine lorsque l’utilisateur retenait sa respiration pendant quelques secondes. Le point de coupure entre fumeur et non-­‐fumeur diffère en fonction de la réglementation locale. Les niveaux de coupures créés par Bedfont sont à prendre en tant que guide uniquement; voir « Système de feu de circulation prédéfini », ceux-­‐ci peuvent être modifiés en ligne avec votre réglementation locale dans le menu des paramètres. Effectuer un test ambiant – Seulement Micro+™ Avec le Micro+™ Smokerlyzer®, l’air ambiant peut être analysé pour vérifier la concentration de CO dans l’environnement. Ceci est particulièrement utile dans le cas où un patient qui est signalé comme étant non-­‐fumeur et obtient un résultat correspondant à celui d'un fumeur, suggérant ainsi une exposition au CO ambiant. Effectuez le test ambiant en appuyant sur l’icône ; le Micro+™ effectuera un test ambiant en temps réel. Pour annuler le test, appuyer sur

Bedfont ne recommande pas que seul le test ambiant soit utilisé comme une mesure du tabagisme passif, car il est possible que des niveaux élevés de CO provenant d’autres sources puissent fausser les données. L’embout buccal doit être remplacé après chaque test respiratoire. 7. Il faut se laver les mains pour éviter les contaminations croisées. 8. Ne tenter pas de modifier l’appareil de quelque façon que ce soit, ou l’utiliser avec des accessoires non spécifiés par le fabricant. Toute tentative invalidera la garantie et peut compromettre la sécurité de l’appareil. 9. Sur demande, Bedfont peut former les personnes qualifiées pour l’entretien du dispositif. 10. En maintenant le bouton de réinitialisation pendant 30 secondes le dispositif se réinitialisera complétement, ceci effacera toutes les données enregistrées et rétablira tous les paramètres aux valeurs par défaut. Après une réinitialisation, il faut vérifier l’heure et la date et étalonner le dispositif avant utilisation. Nettoyage 4. Les produits de la gamme Smokerlyzer® sont moulés avec la technologie SteriTouch® pour la filtration des bactéries et le contrôle contre infections. Cependant, Bedfont recommande d’essuyer le dispositif avec un produit adapté, tel que les lingettes nettoyantes sans alcool fournis par Bedfont. Le piégeur Dpiece ne doit pas être stérilisé. Il se change tous les mois. 5. Lors du nettoyage de l’analyseur, n’utiliser jamais d’alcool, des agents contenant de l’alcool ou d’autres solvants organiques, car ces vapeurs peuvent endommager le capteur de CO à l’intérieur de l’appareil. 6. En aucun cas, l’appareil ne devrait être pas immergé ou éclaboussé par un liquide. 20 Entretien courant 6. Remplacer les piles lorsque l’icône indique que la pile est vide 7. Bedfont recommande de retirer les piles lorsque le dispositif n’est pas utilisé pour éviter tout risque de fuite et de corrosion. 8. Il faut remplacer le piégeur tous les 30 jours ou s’il est visiblement souillé ou contaminé. Le piCO™, piCObaby™ et Micro+™ Smokerlyzer® vous aviseront lors du démarrage si vous devez remplacer le piégeur, vous verrez l’icône « Remplacer le piégeur D ». 9. Le capteur doit être remplacé tous les cinq années. 60 jours avant le changement du capteur, le «Compte à rebours de remplacement du capteur» affichera la date à laquelle le capteur doit être remplacé. Ceci peut être ignoré en appuyant sur l’icône jusqu’à ce que la date de remplacement arrive; à ce moment l’icône « Remplacer capteur » s’affichera. Si vous avez reçu la formation d’entretien par un ingénieur approprié de Bedfont, vous pouvez le remplacer votre-­‐même. Sinon, vous pouvez nous retourner le dispositif. 10. Sur simple demande, Bedfont mettra à votre disposition les renseignements techniques supplémentaires ; veuillez contacter Bedfont ou votre revendeur local. Dépannage L’appareil échoue le démarrage: Si l’appareil échoue à démarrer, remplacer les piles. Assurez-­‐vous que les piles sont bien placées dans le bon sens dans le compartiment. Résultats élevés pour un non-­‐fumeur: Si un non-­‐fumeur obtient des résultats élevés lors du test respiratoire, les explications sont les suivants : 6. Un niveau élevé de CO ambiant (peut-­‐être dû à un appareil à combustion défectueux ou à des gaz d'échappement) 7. Les effets du tabagisme passif 8. L’appareil est contaminé par l’alcool ou une autre matière organique. 9. Le patient peut avoir une concentration élevée d’hydrogène dans sa respiration (Intolérances alimentaires). 10. Le capteur pourrait avoir dérivé et se trouve hors spécifications (voir en dessous). Le capteur a dérivé hors spécifications: Les produits Smokerlyzer® sont calibrés par Bedfont avant livraison, cependant si vous croyez que l’appareil est défectueux, nous vous recommandons d’essayez avec un autre appareil pour faire la comparaison. Alternativement, vous pouvez vérifier le dispositif en utilisant le gaz étalon de Bedfont, ou de le renvoyer chez Bedfont. La vérification à l’aide de gaz requiert du monoxyde de carbone à 20 ppm dans l'air de Bedfont et suivre la procédure ci-­‐dessous. 7. Assurer que la vanne de régulation est en position d'arrêt. 8. Visser la vanne de régulation avec le débitmètre sur la bouteille de gaz. La meilleure façon est de visser la bouteille avec la vanne dans la main gauche. 9. Appuyez sur l’icône ou
de l’écran pour commencer un test respiratoire. 21 10. Lorsque vous êtes invité par l’icône à « Expirer », ou
, ouvrez la vanne de régulation et laissez le gaz s’écouler à 1 litre par minute. 11. Faire circuler le gaz pendant le test respiratoire en surveillant le débit. 12. Si la valeur affichée est moins de 15ppm ou plus élevée que 25ppm, arrêtez le test et étalonnez le dispositif en suivant les étapes ci-­‐dessous, ou le renvoyez chez Bedfont pour l’étalonnage. Suivez les instructions de l’écran pour compléter l’étalonnage. 15. Le débit de gaz doit être réglé à 1,0 litre par minute. 16. Si l’étalonnage a réussi, l’écran affichera l’icône

Logiciel COdata+™ -­‐ branchement au PC Branchez le câble dans le port USB de l’ordinateur et le port de connexion USB en haut de l’appareil. Avant de démarrer le logiciel, assurez-­‐vous que le Smokerlyzer® est raccordé à l’ordinateur et qu’il est allumé. Double-­‐cliquez sur l’icône COdata+™ de l’ordinateur pour démarrer le logiciel. Referez-­‐
vous à la documentation fournie pour savoir comment utiliser le logiciel COdata+™. Procédure de retour Si le dispositif nécessite un entretien ou une réparation, veuillez contacter le service après-­‐vente de Bedfont ou votre revendeur local avant de retourner le dispositif. 8. Un fois le numéro de série de l’analyseur et une description défaut fournis, nous vous donnerons un numéro de retour. Lors du retour d’un appareil, assurez-­‐vous que le numéro de retour et tous les détails, y compris les numéros de téléphone, l’adresse de retour soient clairement indiqués. 22 9. L’appareil doit être décontaminé avant de son retour conformément à la réglementation locale. Bedfont peut fournir un certificat de décontamination que vous devez compléter et fixer à l'extérieur du colis. Ne pas le faire imposera la procédure de décontamination par Bedfont et retardera votre service/réparation. 10. Bedfont conseille d’utiliser votre transporteur et assurer vos produits contre les risques de perte ou de dommage lors du transit. Nous vous enverrons la confirmation de réception une fois la marchandise reçue. 11. Après vérification, un rapport d’ingénieur et un devis pour la réparation vous seront envoyés avec un formulaire d’acceptation. Complétez le formulaire et assurez-­‐vous que le « numéro de commande officielle » soit précisé. Veuillez contacter notre service après-­‐vente si vous ne pouvez fournir un « numéro de commande officielle ». 12. Si le défaut est couvert par la garantie, voir « Garantie » dans ce manuel, Bedfont réparera retournera l’appareil avec un rapport d’ingénieur, gratuitement. 13. Si vous refusez de faire la réparation, des frais de traitement de dossier seront facturés. 14. Vous recevrez votre matériel une fois que Bedfont a reçu les documents demandés.
